ELIGIBILITY & ENROLLMENT
ELIGIBLITY In order to be eligible for benefits you must work a minimum of 30 hours per week (130 hours per month) in a regular full-time position. Coverage will begin on the first day of the month following your hire date. If you are not currently eligible for benefits, but in the future your employment status changes to an eligible class, you will be allowed to join the plan on the first of the month following your status change.
HOW TO ENROLL You can enroll for coverage within 30 days of your date of hire, or during the annual open enrollment period. All enrollments and changes must be completed through ADP’s Employee Self-Service Center at workforcenow.adp.com or through your ADP mobile app.
ELIGIBLE DEPENDENTS Our benefit plans are available to you and your family members. You can enroll yourself, your legal spouse, and eligible children. Eligible children under the plan include biological, adopted, or stepchild(ren) up to the age of 26. If your child is disabled before the age of 26, they may be eligible for continued coverage while disabled beyond age 26.
WAIVING COVERAGE If you elect to waive your coverage options through Barney Trucking, you still must act. You must provide a reason for waiving coverage in your ADP employee benefits portal. Please keep in mind that you will not be allowed to enroll in any of the offered plans if you later change your mind unless you experience a qualified event (see the next page). You will be allowed to enroll during the next Open Enrollment period if you are still eligible.
NEARING MEDICARE ELIGIBILITY? Are you or your spouse nearing Medicare eligibility age? If so, there are important things you should know about how your employer-sponsored plans integrate with Medicare. Reach out to your HR department or the SolV Benefits Advocacy Team (BAT) for more information about what steps you should take if you are, or will be, eligible for Medicare.

ELIGIBILITY & CHANGES
MAKING CHANGES If you experience a qualifying event such as marriage, divorce, birth/adoption of a child or you lose other group coverage you have 30 days to notify Human Resources and make changes to your elections.
HIPAA SPECIAL ENROLLMENT RIGHTS The Health Insurance Portability and Accountability Act of 1996 (HIPAA) provides employees additional opportunities to enroll in a group health plan if they experience a loss of other coverage or certain life events. If you are declining coverage at this time for either yourself or your eligible dependents, you may be able to enroll yourself and/or your eligible dependents in coverage at a later date if there is a loss of other coverage. You must enroll and provide the required supporting documentation within 31 days of the date your other coverage ends. In addition, you may be able to enroll yourself and your eligible dependents if you have a qualifying life event (e.g. change in your marital status, birth or adoption of a child, death of dependent or change in employment status.) You must enroll and provide the applicable required supporting documentation within 31 days of the qualifying life event. REDUCTION OF HOURS If you experience a loss in hours and you are not regularly working the required 30 hours per week to maintain eligibility, you will lose coverage. You will be eligible for continuation of coverage when applicable.
IF YOU LEAVE YOUR JOB In the event that your employment with your employer ends, qualified beneficiaries will be offered COBRA continuation coverage. You will receive election paperwork and be given the opportunity to continue to cover yourself or your previously enrolled dependents on the plan.

MEDICAL MEDICAL INSURANCE Barney Trucking offers one Health Savings Account (HSA) compatible high-deductible health plan that is administered by Health Now (HNAS) and utilizes the 6 Degrees network of doctors and hospitals. The open access plan allows you the freedom and flexibility to select a provider of your choosing. It is important to remember that, when you are choosing a provider, your out-of-pocket expenses will be less by choosing a recommended provider. To find a preferred provider, see page 8. Below is a brief description of the components that make up your Barney Trucking medical plan.
Health Now (HNAS) is the Third-Party Administrator (TPA) for the Barney Trucking medical plan. HNAS provides the following for Barney Trucking employees: •Customer service for ALL medical plan related questions •Medical management administration •Online portal and mobile app •Medical ID cards •Evidence of Insurability (EOB) documentation
Barney Trucking is partnering with 6 Degrees Health to assist you with reviewing and evaluating every claim to ensure your healthcare costs are paid at a fair and reasonable price. 6 Degrees Health provides claim review, member support and advocacy, and expert legal services. If you ever receive a bill that does not match the EOB from Health Now (HNAS), contact Health Now and they will connect you to 6 Degrees Health for help. One of their dedicated Member Service Advocates will work with you through the resolution of your billing issue.
Southern Scripts is the Prescription Benefits Manager for the Barney Trucking health plan. The Southern Scripts pharmacy network consists of independent and retail pharmacies. Please visit southernscripts.net/members to access the Network Pharmacy Locator Tool. Southern Scripts also has a convenient mail order pharmacy program where you can get your prescriptions delivered directly to your home. Southern Scripts RxCompass will be available to identify savings opportunities, if you qualify. Depending on the prescriptions you fill, you will receive calls directly from the RxCompass concierge team to assist you.
Teladoc is included with the Barney Trucking medical plan, and available to all participating members at no cost. Enrolled members will have access to free telephonic doctors’ appointments, including behavioral health. Teladoc members have free access to U.S. board-certified doctors who can resolve many of your medical issues, including writing prescriptions via phone or online video consults. Your 2023 medical plan uses an "Open Access Network". This means that your plan will provide coverage regardless of what provider you see. The plan will pay a fair market value to your provider and you should not pay any balance bills (bills sent to you in excess of what the plan has paid) without contacting 6 Degrees first. Your provider may have questions about your plan, here are some important things to remember: • If you provider has questions about your plan, or says that they do not accept it, ask them to call Health Now (HNAS), our administrator, at 855.438.2110. If they refuse, call the SolV BAT line at 833.476.5848. • If you are asked who your health insurance carrier is, you should state that you are enrolled in your employers self-funded health plan and the administrator is Health Now (HNAS). • Your provider may want to know if your plan is a PPO or an HMO, you can tell them that it is an open access plan, and not an HMO. • Your plan does not include a static network. It covers any provider. WHAT IS AN EOB? EOB stands for “Explanation of Benefits.” This is a document that is available to you after your claim has processed. It shows you what you owe (such as your deductible, copay and/or coinsurance) after plan benefits have been applied and the insurance has paid, according to the allowed amount.
WHAT IS A BALANCE BILL? A balance bill is any amount a provider is billing you that is more than what your EOB says you are responsible for, after the insurance has paid. 2023 BENEFITS GUIDE

PRESCRIPTION BENEFITS WITH SOUTHERN SCRIPTS FirstChoice is the preferred pharmacy network of Southern Scripts, offering access to reduced prescription costs at participating FirstChoice pharmacies across the nation. A participating FirstChoice pharmacy offers, on average, a lower cost on medications for covered drugs than a standard (nonpreferred) pharmacy. FirstChoice consists of both independent (local/community) and retail (national/regional) pharmacies. It’s easy to transfer your prescriptions to a FirstChoice pharmacy! 1. Locate a FirstChoice pharmacy by following the steps above • Remember: FirstChoice (preferred) offers, on average, a lower cost on medications for covered drugs than a standard (non-preferred pharmacy. 2. Contact your new participating FirstChoice pharmacy • Inform your new participating FirstChoice pharmacy that you want to transfer your prescriptions from your old pharmacy. You will need the name, strength, and prescription number of each prescription, along with the phone number of your old pharmacy. You can accomplish this by calling, visiting the new pharmacy in person, or if your new pharmacy offers the technology, by using online services such as a website or the mobile app. 3. What information will the pharmacy need to facilitate the transfer? • Your new pharmacy will need your health insurance information, and perhaps some personal information as well. Typically, your new pharmacy will request: • Health Insurance card • First and last name • Date of Birth • Address • Phone number • Possible allergies you may have 4. How long will it take to complete the transfer? • The amount of time your new pharmacy needs can depend on how many prescriptions need to be moved over, possibly 1-3 business days. When the time comes to pickup your prescription(s), please bring your insurance card with you to the pharmacy.

HEALTH SAVINGS ACCOUNT ELIGIBLITY If you enroll in the Barney Trucking medical plan you are eligible to contribute to a Health Savings Account (HSA). HSAs were created by the federal government to give people a tax-advantaged way to pay for medical expenses and save for future needs. An HSA is tax-advantaged because you generally are not taxed at the federal level on contributions, earnings, or withdrawals.
You are eligible to fund an HSA if: • You are enrolled in the Barney Trucking Health Now HSA plan
You are NOT eligible to fund an HSA if: • You are covered by a non-HSA eligible medical plan, health reimbursement arrangement, or health care FSA. • You are eligible to be claimed as a dependent on someone else’s tax return • You are enrolled in Medicare, TRICARE, or TRICARE for Life

LIFE INSURANCE BASIC LIFE AND AD&D INSURANCE
Life and accidental death and dismemberment (AD&D) insurance is an important element of your income protection planning, especially for those who depend on you for financial security. For your peace of mind, Barney Trucking provides complimentary, no cost basic life and AD&D insurance through Mutual of Omaha to all regular full-time employees. Please be sure to review your beneficiary information and update if needed through the ADP Employee Self-Service portal. A beneficiary is the person (or people, estate, trust, etc.) to whom benefits will be paid to in the event of your death. You may change your beneficiary at any time during the plan year.

Barney Trucking provides you the option to purchase supplemental life and AD&D insurance for yourself, your spouse, and your dependent children through Mutual of Omaha. You must purchase supplemental coverage for yourself in order to purchase coverage for your spouse and/or dependents. If you elect supplemental coverage when you’re first eligible to enroll, you may purchase up to the guaranteed issue amount(s) without completing a statement of health (EOI: evidence of insurability). Coverage will not take effect until approved by Mutual of Omaha. • Employee: $10,000 increments up to 5x annual salary or $500,000; Guarantee Issue: 5x annual salary or $150,000, whichever is less • Spouse: $10,000 increments up to $250,000, not to exceed 100% of the employee’s election; Guarantee Issue: $30,000 • Dependent children: $2,500 increments up to $10,000, not to exceed 100% of employee’s election; Guarantee Issue: $10,000 Tobacco/non-tobacco rates apply. SHORT-TERM DISABILITY INSURANCE VOLUNTARY SHORT-TERM DISABILITY (STD) Disability insurance benefits replace a portion of your income if you are unable to work for a period of time due to a qualified, off-the-job injury or illness. No matter how your medical expenses are currently covered, you should have an insurance plan that helps cover everyday living expenses if you are not able to work. WELLNESS PROGRAM Barney Trucking believes our employees are our greatest asset. We also believe the greatest asset any individual can have is their health. We value your health and strive to continue to identify programs and tools that can assist you in managing your overall well-being. The Barney Trucking Wellness Program is a voluntary wellness program, and all enrolled employees are invited to participate. Employees who choose to participate in the health evaluation will receive an incentive of $50 per month ($600 annually) in premium savings. An additional incentive of $50 per month ($600 annually) in premium savings will be awarded to employees who test negative for nicotine/cotinine or tobacco users who enroll and complete Marquee Health’s UBreathe Program. Please be aware, Barney Trucking will be testing for the presence of nicotine/cotinine during the biometric screening to verify tobacco use. All employees interested in the wellness reward must go through the onsite health screenings or complete a screening through their Primary Care Physician or a lab facility by mid-December. TOBACCO SOLUTIONS POLICY Our employees are the lifeblood of our company. The number one item of importance to us is the physical safety of our team members. With this in mind, we have created a policy designed to protect the lives of our employees and their families. It is evident that one of the best things anyone who is a tobacco user can do for their health is to quit the use of tobacco. Quitting tobacco not only has immediate health benefits but it also has direct and indirect cost savings that can benefit tobacco users, their families, and the company at large. We believe in helping and providing tools for individuals to quit tobacco. To encourage Barney Trucking employees to live a healthier life and to combat some of the costs associated with tobacco use, Barney Trucking offers a tobacco-free program. We also offer a few Tobacco Cessation Program (TCP), the UBreathe Program, through Marquee Health. We have a partner in this effort to bring an effective program to our employees. By meeting the program requirements and deadlines listed below, a tobacco user can earn the same incentive as a non-user. We care about you. The aim of this program is to help individuals stop using all tobacco or nicotine products by combining an educational, therapeutic and incentive based model. Tobacco-Free Reward • Any employee enrolled in the Barney Trucking medical plan who is not a tobacco user will receive a premium reduction of $50 a month, beginning January 1. • If an employee is identified as a tobacco user, he or she can complete the company provided TCP defined below as a means for earning the wellness reward. • Upon successful completion of the TCP by the given deadline, the employee will retroactively be reimbursed the surcharge amount that was paid prior to completion and will earn the premium reduction for the remainder of the year. • Tobacco users who choose not to, or fail to, participate in the TCP will not receive the premium reduction for the new plan year. Tobacco-Use Testing • To be eligible for the Tobacco-Free premium reduction, all employees enrolled in the company medical plan must verify their tobacco-use status via blood draw during the Marquee Health evaluations. • The nicotine & cotinine test will be part of the biometric screening blood panel through Marquee Health, which must be completed by mid-December. • The test will detect the nicotine metabolite in the blood if any nicotine containing products have been used 10 days prior to the testing date. • If an employee fails to complete the biometric screening by the given deadline, they will be considered a tobacco user and will not receive the premium reduction. • If the employee become a tobacco user during the plan year: • It is the individual's responsibility to report this change to Human Resources within 30 days • The individual will be given an opportunity to complete the TCP within 4 months. • If the individual fails to complete the TCP by the given deadline, he/she will no longer receive the premium reduction for the remainder of the year. Reasonable Alternative Standard Employees who are tobacco users may participate in the UBreathe Program through Marquee Health to receive the same reward as non-tobacco users. • Individuals have until mid-January to enroll in the UBreathe Program and must complete 6 health coaching sessions by April 30th to receive the wellness reward. • Upon successful completion of the TCP by the given deadline, the employee will retroactively be reimbursed the surcharge amount that was paid prior to completion and will earn the premium reduction for the remainder of the year. • It is not mandatory for an individual to quit using tobacco in order to receive the discount. Although quitting is encouraged, participants will receive the premium reduction upon completion, regardless of tobacco status. • If the individual fails to complete the TCP, they will not longer be eligible for the premium reduction for the remainder of the plan year. 2023 BENEFITS GUIDE

GLOSSARY OF TERMS AD&D (Accidental Death & Dismemberment)
A plan that provides benefits in the event of an accidental death or dismemberment (generally, an accident that results in death, loss of part of the body, or the loss of the use of part of the body).
Beneficiary
A person designated by a participant, or by the terms of an employee benefit plan, which is or may become entitled to a benefit under the plan.
COBRA
Federal law (Consolidated Omnibus Budget Reconciliation Act of 1985) requiring certain employers that offer group health plans to provide continuation coverage to employees and their dependents who incur certain qualifying events.
Co-Insurance or Cost Sharing
The portion of covered health care costs for which you are financially responsible. Coinsurance does not include deductibles or copays.
Co-Payment or Copay
A set amount you pay out of pocket for a particular service. The plan pays the balance.
Deductible
The out-of-pocket amount you must pay each plan year before the plan pays for eligible benefits.
Evidence of Insurability (EOI)
Many insurance companies require prospective clients/ individuals to prove that they are in good health and are therefore good insurance risks before the company will cover them.
Explanation of Benefits (EOB)
A statement from a plan explaining what portion of a claim was paid.
Generic
Your prescription drug copay depends on the class or group of your prescribed medication. A generic drug generally has the lowest copay level. A generic drug is one that is no longer produced only under a brand name. Once a drug’s patent expires, many companies can begin to manufacture “generic” versions of a previously brand- name-only drug. Generic drugs are identical to brand-name drugs in chemical makeup (“active ingredients”), usage, strength and dosage. They are regulated and approved by the FDA just like brand-name drugs; however, they are much less expensive.
HIPAA Authorization
Under HIPAA, a document that authorizes the use or disclosure of an individual’s Protected Health Information by a Covered Entity for any purpose described in the document and meets specific requirements.
Negotiated rates
The costs for health care services negotiated between the insurance carrier and in- network health care providers. Negotiated rates are usually less than usual, customary and reasonable (UCR) charges.
Non-preferred brand
Your prescription drug copay depends on the class or group of your prescribed medication. A nonpreferred brand-name drug generally has the highest copay level because it is not on the plan’s list of preferred drugs.
