Honorable Prime Minister Shri Narendra Modi's clarion call for Heal in India by India is a timely, visionar y, compassionate, inclusive and much needed universal health care outreach. Months ago, while global healthcare was stuck dealing with pandemic. This has resulted in piling up of native elective surgeries. It is also sad to note that due to pandemic induced inflation, corresponding as well as average global per capita income has considerably gone down. To set right this demand-supply mismatch and ward off resultant inflationar y/premium healthcare pricing trends, reasonably priced Indian healthcare deliver y ser vices could offer huge respite to global healthcare governance and management. Symposia, Conferences, Global/ Regional Healthcare Outreach programs in coordination with foreign embassies and consulates and Indian embassies and consulates would indeed be a game changer towards MVT projection of $13 billion by 2026 ! KRSNAA LEVERAGES AI TO DELIVER QUALITY DIAGNOSTICS TO THE RURAL HINTERLANDS OF INDIA • India’s largest diagnostics player has joined hands with SigTuple to use AI powered technology for automated microscopy processes • The tele-pathology reporting centre in Pune will use AI100 to digitize its entire network of pathology labs, to reduce the turnaround time • Krsnaa Diagnostics already has India’s largest tele-radiology centre in Pune spread across 4,500 sq ft

rsnaa Diagnostics, India’s largest diagnostics player and pioneer in tele-radiology has made a significant step to leverage Artificial Intelligence (AI) to improve the diagnostics outcome. Krsnaa Diagnostics will set up tele-pathology reporting centre in Pune, to use AI100 to digitize its entire network of pathology labs. For this, Krsnaa Diagnostics has tied up with SigTuple to use AI powered technology to automate the manual microscopy process across its entire pathology network. The tie-up is expected to significantly reduce the turnaround time, standardize reporting quality and increase efficiency. Globally, artificial intelligence assisted digital microscopy is the way forward for pathology. Digital microscopy is the process in which a physical sample is digitally imaged through a microscopic lens. This makes it possible for pathologists to review them remotely, without having to ship the samples to the reference laboratory, as the digital conversion can happen at site. SigTuple AI100 is the premier solution for AI assisted

digital pathology. It is also the only digital pathology solution available, which is economical and robust enough for wide scale adoption in emerging markets. Commenting on the collaboration, Tathagato Rai Dastidar, Founder & CEO of SigTuple said, “SigTuple started in 2015 with a singular goal – to make quality healthcare more accessible, more accurate and more affordable. AI100 was built to solve the problem of digital pathology with that aim in mind. We are very excited to see that vision coming true in real life, solving real world problems in rural India, in partnership with KRSNAA Diagnostics. We hope to replicate this success in the rest of the world very soon.” Highlighting on the positive impact, Dr. Ravinder Sethi, Chief Operating Officer from Krsnaa Diagnostics said, “We are glad and proud to collaborate with SigTuple and with the tie-up, Krsnaa Diagnostics has moved a step closer in making healthcare accessible for all. We are a staunch believer of technology enabled quality diagnostics at affordable costs. The SigTuple’s AI100 will be deployed across our laboratories. To begin with, we have commenced deployment in Punjab, Himachal Pradesh and Maharashtra.” Echoing similar opinion, COO of Pathology Services at Krsnaa Diagnostics, Dr. Manish Dattatreya Karekar, said, “One of the major challenges that any diagnostics provider faces in setting up quality diagnostic centres outside of the large urban areas, is the availability of pathologists. In the absence of a pathologist at site in laboratories outside urban areas, these samples need to be shipped to central reference laboratories for review. This tie-up will help the pathologists work efficiently and bring standardization in reporting processes".

IIT GUWAHATI SCIENTISTS DEVELOP A STRATEGY TO DELIVER CHEMOTHERAPEUTIC DRUGS SPECIFICALLY TO CANCER CELLS

T

he developed approach would allow the development of drug carriers for chemotherapy with enhanced efficacy and negligible side effects. Indian Institute of Technology Guwahati researchers have developed a new strategy to deliver chemotherapeutic drugs specifically to the cancerous cells in a patient’s body. The pathbreaking results of this research have been published in prestigious journals of The Royal Society of Chemistry including ‘Chemical Communications’ and ‘Organic and Biomolecular Chemistry’. The research papers have been co-authored by Prof. Debasis Manna, Department of Chemistry, IIT Guwahati, along with his research scholars Mr. Subhasis Dey, Ms. Anjali Patel, and Mr. Biswa Mohan Prusty, among others. Anticancer activities were carried out in collaboration with Prof. Siddhartha Sankar Ghosh and Ms. Plaboni Sen from the Indian Institute of Technology Guwahati and Prof. Arindam Bhattacharyya and Mr. Soumya Chatterjee from Calcutta University. A part of the ongoing research was recently highlighted as the cover page of Chemical Communications. The problem with existing chemotherapeutic drugs is that they kill healthy cells of the body in addition to cancerous cells,

14 October 2022  w w w. me d g a te to d a y.co m

leading to numerous undesirable side effects. In fact, it is believed that cancer deaths are as much due to the side effects of chemotherapy as the disease itself. There is worldwide research to overcome the drawbacks of secondary toxicity of chemotherapeutic drugs. Some strategies that are being explored include targetspecific delivery of the drugs and ondemand delivery of appropriate drug doses to cancerous cells/tissues. Explaining his research Prof. Debasis Manna, Department of Chemistry, IIT Guwahati, said, “We have two needs in the development of chemotherapy drugs – the drug must be targeted at the cancer cells, the drug must be released by an external trigger whenever it is required,” To meet the above needs, the molecule developed by the research team has four special features. •

The first feature is that the molecules assemble to form hollow spherical shells in water These shells that are ten-millionth of a meter in size can be used as a minuscule container for the drug molecule

The second characteristic is that the molecule has a part (the acetazolamide ligand) that specifically binds to cancer cells and not normal cells

The third feature of the molecule is that it has a photocleavable linker moiety that is responsive to infrared light and breaks the shell when exposed to IR

Molecule also contain a dye moiety (cyanine-3) which is also useful for both fluoresce and scattering-based imaging to visually monitor the entire process.

o Create Awareness of Heart Care and HeartHealthy Lifestyle

In a bid to create awareness and promote a healthy heart among the masses and to educate the common man to fight against cardiovascular disease (CVD), Medicover Hospitals of Navi Mumbai held a mini walkathon from the service road to three monkey points and took a pledge on Thursday, 29th September, on the occasion of World Heart Day. Sudden Cardiac Death (SCD) is a catastrophic event and it is contributed to 10.3% of overall mortality. The majority of SCD cases will occur in the general population who are not known to have heart disease. With aiming to make awareness about cardiovascular disease and early identification of cardiac disease. a mini walkathon was organised by Medicover Hospitals in Navi Mumbai. The walkathon was flagged by releasing the heart-shaped balloons. Along with the hospital staff, doctors participated in the walkathon, each carrying messages on how to prevent heart disease and take care of the heart. The Hospital staff and doctors also took a pledge for heart care and a heart-healthy lifestyle. The hospital organised free cardiac screening camps in collaboration with nursing homes, clinics, societies, diagnostics,

and corporates across Navi Mumbai. During these camps, the hospital provided free screening including ECG, RBS, and doctor consultation for heart care. Hospital has organised community health talks to raise awareness about cardiac disease. The webinars were held by hospitals to generate awareness among younger people about cardiac health. “Though peak prevalence of SCD is in a geriatric group, the age trend is now changing in the last few years. It is rising in prevalence from 30 years and peaking at 50 to 70 years. The existence of a large substrate of undiagnosed coronary artery disease in a younger population contributing to SCD is a major health issue in India that merits focused designing of effective preventive strategies. Therefore, we are focusing on the early diagnosis of cardiac disease and on-time treatment”, said Dr. Brajesh Kunwar, Head of Cardiology Department, Medicover Hospitals, Navi Mumbai. “This is due to the unhealthy lifestyle. We organized a 20 minutes walkathon which highlighted the importance of a brisk 20-minute walk each day could reduce the risk of heart disease. ”, said Dr. Anup Mahajani, Consultant Interventional Cardiologist, Medicover Hospitals, Navi Mumbai.

hat is the relationship between hypertension and heart diseases?

According to an article published by the National Library of Medicine, high blood pressure (BP) is one of the most important risk factors for cardiovascular disease (CVD), which is the leading cause of mortality. An individual with hypertension is almost 10 times at a higher risk of being affected by a heart attack, stroke or paralysis, brain haemorrhage, or chronic kidney disease such as kidney failure at some point in their life. That’s because the blood throughout our body exerts a certain pressure and is directly linked to our organs, and when the blood MBBS(Hons) Gold Medalist pressure exceeds the normal range, it may have adverse effect on our health. ABPM is another method that allows the doctor to assess the patient’s blood pressure over an extended period outside the doctor’s office. This method provides overall blood pressure levels and offers a more complete picture of the patient’s condition, to ensure appropriate treatment and control of elevated blood pressure. Some risk factors such as family history that cannot be modified while other risk factors like high blood pressure can be modified by controlling the habitual risk factors such as smoking, eating unhealthy diet or stress. An individual will not necessarily develop cardiovascular disease if they have a risk factor. But the more risk factors one has, the greater the likelihood that they will, unless they take action to modify your risk factors and work to prevent them. Any advice to people on World Heart Day? Some of these unhealthy lifestyle habits and choices have been a problem in this country. one needs to seek regular, routine medical help to identify the risk factors, lay out a plan for how to deal with those risk factors with your health care provider, and then work hard to adhere to that plan. This world heart day, my advice to the readers would be to review their everyday choices and start small. Cut down on sugary beverages and choose water or unsweetened juices instead. Swap sweet, sugary treats for fresh fruit as a healthy alternative. Include fruits and vegetables in your diet.

SWASTHYA SAATHI & THE MIRACULOUS TRAUMA CARE AT MEDICA TOGETHER SAVES THE RIGHT LIMB OF A DAILY WAGE EARNER FROM HASNABAD

M

edica Group of Hospitals, the largest private hospital chain in Eastern India, at its flagship facility Medica Superspecialty Hospital in Kolkata conducted a lifesaving trauma care treatment on a 30-year-old male from Hasnabad, Nandanpur Kolkata. The team of experienced doctors from the GI Surgery department and Medica’s care team joined together for this case. The team put in their combined efforts under the leadership of Dr. Udipta Ray, Senior Consultant GI Surgery, Medica Superspecialty Hospital along with the care team of Medica ensured to achieve this momentous feat on 13th September 2022. Mr. Sujoy Chakraborty, 30, of Hasnabad, Nandanpur, earns his living by loading and unloading trucks equipped with construction materials. He was being driven back home on a tempo when the driver lost control and crashed the tempo into a metro construction site near Medica. Sujoy was severely injured and was screaming in immense pain. “It was late at night, the roads were empty, and I was driving the tempo at rapid speed. I somehow lost control, and the car was immediately crushed against some iron rods from the metro construction site, one of which went through Sujoy’s upper side of his right arm,” the driver explained. After seeing his condition, he was rushed to the operation theatre & was instantly provided with adequate ice fluids and other medication for pain relief and to stop the bleeding. A large portion of the rod was protruding on each side of the wound, causing extensive injury to the surrounding tissues, muscles, and skin. Due to the exigency, Medica had to arrange for a metal cutter to cut one end of the rod very near the skin so that the

that the patient was stable, the wound was healthy, and there was no bleeding, the appropriate dressing was initiated, and the patient was discharged with physiotherapy.

other portion could be removed easily. The rod was gently removed, taking complete control of the joint, the primary bone of the upper arm, muscles, and skin to prevent these tissues from any further damage. However, the skin was lacerated and abraded, and the edges of the skin were completely burnt due to the heat of the cutter. Later when the rod was removed, the bleeding vessels from the muscles were all secured, and the dead/damaged tissues that were permanently destroyed during the accident were all removed, leaving fresh tissues to recover. The bleeding was completely stopped, and as much muscle repair as feasible was performed. Because the wound had dust, dirt, iron particles from the rod, and even the patient’s clothes within, it required thorough cleaning to avoid infections. An adequate functional assessment was performed following the successful removal of the rod and proper wound care, indicating that the patient recovered well from the surgery. To avoid any loss of muscle activity, early physiotherapy was initiated. The bleeding was controlled and prevented even after the post-operative period with proper gauge piece packing. After ensuring

Dr. Udipta Ray, who led the operation, said, “Sujoy was in a state of shock after seeing the pool of blood when he was brought to Medica. He screamed in pain. This was one of the most serious cases I had seen in a long time. To begin with, being a doctor, and working as an expert welder was a very challenging situation for me. Second, the removal of the rod could have further damaged the skin because it was already burnt from the heat of the cutter, and third, the rod could have injured the main bone of the upper arm, the ‘humerus,’ and caused a dislocation of the joint because any manipulation of the rod could only be done with the patient under full anesthesia. As a result, preventing the damaged adjacent joints and bones was a critical issue and it was taken care of during the entire procedure. We are pleased to see Sujoy’s quick recovery despite the difficulties.” He further added, “Because of his financial circumstances, he was worried while being admitted to the hospital. We wasted no time because he had his Swasthya Saathi card and promptly admitted him to the scheme. This did not offer him with adequate comfort from his physical suffering, but it did provide him with mental relief.” The patient is said to be doing well at the time of the follow-up, with full functional recovery of the arm. The procedure was not only lifesaving, but it was also a remarkable limb-saving surgery at Medica. Saving the right arm from amputation makes a huge difference for a daily earner.

HIGHLY EXPERIENCED SURGEONS OF INDIA GAVE A NEW LEASE OF LIFE TO A 106-YEAR-OLD PATIENT SUFFERING FROM A BLOOD CLOT IN THE BRAIN heart and lung disease and clogged arteries may make one more vulnerable to experience side effects or complications. The patient’s family had almost lost hope of a proper treatment for Sriram when they consulted a doctor at Asian Hospital for further advice and management.

S

riram, a 106-year-old man from Faridabad, Sector 28 successfully underwent a complicated brain surgery performed by a team of surgeons at the Asian Institute of Medical Sciences (Asian Hospital), Faridabad. An otherwise healthy and energetic centenarian, Sriram was bedridden for a month. He was living in a state of unconsciousness and faced great difficulty in even putting a step down to walk. His family members were surprised to see him in this state as he had always led an active life. They took him to a GP, who advised a CT scan of the brain to evaluate the cause of his illness. The scan revealed the formation of blood clots in both parts of the brain which were hindering his day-to-day activities resulting in restlessness. The family had consulted several doctors for adequate treatment and management of his condition, but to no avail. Looking at the age of the patient, the surgery involved multiple risks during or after surgery, likehigh risk of death during surgery, going on ventilator after surgery, prominent being an increased risk of bleeding which in turn can lead to shock, deep vein thrombosis, pulmonary embolism, reaction to anesthesia among others. Besides some common age-related health issues like high blood pressure,

After reviewing the patient’s condition, Dr. Mukesh Pandey, Associate Director & HOD – Neurosurgery, Asian Hospital said, the clots could not be eliminated through medicines and therefore, we decided to go for surgical removal of the clots. While the surgery was not without risks, Sriram’s family members showed great trust in our team and had faith in medical advancement as well. That is how we went ahead with the surgery. Looking back, I feel it was the best thing to have happened.” Sharing details of the surgery, Dr. Pandey added, “we opened both parts of the brain and successfully removed the blood clot, after which Sriram was able to walk the very next day. He is back on track leading a healthy and active life like before.” According to the team at Asian, he will be able to lead a normal life without any further issues. ASSOCIATION OF MEDICAL CONSULTANTS (MUMBAI) PLEDGES SUPPORT TO THE KEM HOSPITAL MARATHON, PARTICIPATES TO RAISE AWARENESS ABOUT THE ERADICATION OF TUBERCULOSIS

T

he Association of Medical Consultants, Mumbai participated in the KEM Hospital Marathon to pledge their support towards ‘Eradication of Tuberculosis’ in the country. On Gandhi Jayanti, the doctor community participated in the marathon for a cause to raise awareness about this deadly disease. India notified more than 2.4 million TB cases in 2019 and continues to have the largest share of the global TB burden, according to the National Health Portal of India. The COVID-19 pandemic drew too much attention in the last few years, side lining other public health issues which have become a global crisis. Tuberculosis is no less of a pandemic and requires urgent attention and faster intervention to avoid the rising death numbers not only in India but globally too. Dr Nilima Vaidya Bhamare, President, Association of Medical Consultants, Mumbai, said, “Tuberculosis is a preventable and curable disease

however as a country we are yet to get a grip on the infectious disease. Lack of awareness about the dangers of TB, accessibility to diagnosis and testing coupled with affordability of treatment are increasing the disease burden in the country. The spread of Tuberculosis can be reduced if we follow preventive measures such as covering of mouth while coughing or sneezing, refraining from spitting in public places, using masks to reduce the transmission of airborne diseases, a practice that can be extended beyond covid-19 and followed for other transmittable diseases. We as an association of forward-thinking medical practitioners are doing our bit by supporting KEM Hospital in the marathon held for this cause.” The marathon witnessed a participation of around 900 runners who chose to run either 10km, 5 km or 3km depending on their stamina and interest. The marathon begun at 5.30 am and ended with a speech from the representative from AMC, Mumbai.

INDIA AS MEDICAL INTERVENTION & WELLNESS HUB

M

edical tourism has been around for centuries. Belief has existed that bathing in hot springs, has therapeutic value for a number of ailments (including chronic skin diseases) and so those who could afford it, especially the ruling classes, would visit such locations seeking cures. Even pilgrimage to shrines and to abodes of miracle workers is a form of medical tourism. The TB afflicted would seek out more salubrious – dry and warm locations to resettle – even if stays were relatively brief.

been on the rise in “poor” countries and this has allowed many to seek more expensive intervention perceived to be either “state-of-art” or treatment through “alternate” forms of medicine.

With journeys becoming easier to undertake, the last five decades have seen a mushroom growth in the volume of traffic for medical reasons. The proliferation began owing to spiralling costs of medical intervention in affluent regions. This brought about exploration of lower cost options elsewhere by patients, doctors and hospitals and even insurance companies. Provided it was feasible to travel and that a solution was readily available abroad, decisions were then made based on the travel cost and treatment cost relations. For a while, affluent nations would display a distinct bias about the quality aspects on offer elsewhere. Word of mouth publicity (such as about the better quality of nursing care in India or Singapore); the generation of analytical scientific reports; awareness creation and promotional activity; the modernizing and “catching-up” that was ongoing in the under-developed world vis-a-vis the developed; has brought about considerable shift in perception. Both Institutions and patients are not as hesitant as before.

Granted that Covid had played its part, but by 2021 over 2,00,000 British patients were waiting for CABG (5,000 for more than a year). It was similar for other interventional procedures – cardiac, GUM, knee and hip replacement. This in itself made a strong case for medical tourism. While the system could provide high-quality health care, this was only done “eventually”. It did not provide timely intervention

Meanwhile, well-to-do classes have 32 October 2022  w w w. me d g a te to d a y.co m

Not all such medical tourism decisions are logical however. A case in point is the UK’s NHS. While the United Kingdom has an enviable Health System, in the National Health Service that seeks to provide healthcare nearly free of cost for all citizens, the NHS has piled up huge backlogs by way of patients waiting for overdue medical intervention.

and so inflicted unnecessary suffering, morbidity, and mortality. Unnecessary! Because competent doctors, enough beds, and adequate equipment and facilities were available in places like India and Thailand for procedures that have become so common as to be “off-the-shelf”. Having analysed this in some depth, take the case of the UK allotting 5,000 CABG level interventions to the medical tourism route. On average CABGs cost £11,000 each in the UK. Including transportation (air-fares, an attendant’s costs and procedure) the average cost for an Indian case would be around £7,000. The resulting saving for the NHS would be of the order £20,000,000. Such savings could then release considerable funds for putting back into the NHS as required. It would have been a win-win outcome for both nations. It was an opportunity missed. It required understanding on the part of the Indian Government – since it involved politicians in the UK – all the way down to the level of local selfgovernance councils.

The Medical Tourism Market: Estimates Global Medical Tourism Market in 2021: US$ 18.7 billion. Compound Annual Growth Rate (CAGR) expected for next decade: 20%. Market size expected in 2030: US$ 107 billion. Medical Tourist traffic in 2017: 14-16 million Medical Tourist Traffic in 2021: ~ 18 million These traffic figures are compiled from those of individual states. Some (such as from the US, Mexico, EU and India) are quite accurate.

Factors that will drive Growth • Cost Differentials for equivalent treatment in different nations. • Dissemination of awareness about what is on offer in destination location. » To elaborate: The larger Indian players enjoy a level of visibility (and created brand images for themselves) in some markets abroad. However, awareness of the full potential of India is low in most parts of the world. » This has to percolate to Governments, Medical Institutions and Bodies, Doctors and the Medical Communities, and finally among Patients. » To move from awareness to acceptance, word of mouth is vital. Thailand is way ahead of India as regards this. • Adoption of state-of-art technology and practices. Catching up with the developed world. • Quality aspects: Such as dedicated nursing and post operative care. • Partners in originating countries: Tie ups with insurance companies for instance. • Synergies: In addition to high quality conventional intervention, India also offers alternate medicine, and a variety of appealing wellness therapies.

Limiting Factors • Competition: A number of different regions will begin to compete with each other for a slice of the medical tourism pie. Not only will this be about prices but also in terms of reputation and brand imaging. A single rogue player can damage perceptions about all others in that destination.

• Post operative needs and their fits with protocols of the origin states. • Changes in health demographics • Capacities & Transportation: While Crete, Turkey and the Virgin Islands happen to be the preferred destination of medical tourists from Europe and the U K, only Singapore, Thailand and India have any real capacities at the moment. Among the latter India is best placed in terms quality hospitals all over the country. Upcountry centres, CMC Vellore for example, may have all the attributes of an excellent health centre but the question is: will someone desperately sick accept the imposition of getting there? Also Air-fares to India exceed those to Thailand. • Innovation or the lack of it: Thailand for instance has positioned itself as the destination for cosmetic surgery – offering instant face-lifts at bargain rates. Thailand generates the highest volume of medical tourist traffic in the world, and this further leads to huge word of mouth publicity. India needs strategy for generating foreign traffic for its larger “boutique” hospitals. Transportation is not confined to air. Medical tourists from Bangladesh to Thailand outnumber those to India many times over. Cheap overland transport forms exist – but these are unsuitable for the unwell. What about Pakistan and Nepal?

SUMMARY • Medical Tourism is one of the fastest growing activities on the planet and presents huge opportunities for Indian players. So is Wellness Tourism. • For such entrants, it is important to develop strategies based on what they have on offer. Such strategies will identify countries of relevance and who they need to partner with in these regions. • Medical Tourism has the potential to earn foreign exchange on a scale comparable to our IT sector and so requires Government support: » By building bilateral relations (as illustrated by the NHS reference given above) » Through organizing awareness building campaigns at identified locations and providing forums to a range of Indian health care businesses including niche and boutique style entities.

W

e all know the field of Healthcare in India is rapidly growing and advancing. The growth is observed not only in the traditional sectors of Hospitals and Pharmaceuticals but also in the newly emerging field of Medical tourism where the demand for Indian healthcare services has increased significantly in the international community as well. With new medical advances in healthcare there is a strong need for innovative practices to overcome challenges faced in this sectors collectively for long-term growth and sustainability. Medical tourism is one of the sectors, which is important to both Healthcare, and Tourism industry of our country. A combination of many factors has led to the recent increase in popularity of medical tourism: exorbitant costs of healthcare in developed nations, ease and affordability of international travel, favourable currency exchange rates in the global economy, rapidly improving technology and standards of care. A combination of three key factors – quality, availability and cost has been key factors in fuelling the phenomenal growth witnessed in the Indian medical tourism industry. Medical tourism services could include complex specialized surgeries such as joint replacement (knee/hip), cardiac surgery, IVF, dental surgery, and cosmetic surgeries. But other types of health care like psychiatry, alternative treatments (example, Ayurveda), convalescent care, wellness etc. are also available. Though the industry has shown increasing potential in the last few years, most of the international patients from Middle East and Europe still prefer countries like Thailand where medical services are at par with India but they prefer it due to the ease of obtaining visa and friendly mind-set and general attitude of the people towards tourists. There is also a need to curate quality destinations within India where differentiated experience that is unique could be provided to the quality-seeking visitors, Quality medical tourism destinations could also help us with sustainable growth that will be innovative, productive and lean. There is also a need to undertake an international marketing campaign to create awareness amongst the global community about the facilities rendered by Indian healthcare institutions. In order to boost medical tourisms strong efforts need to be made towards the same. We at Wadia Hospitals strongly believe in providing quality care at affordable costs for all individuals who visit us including international patients.

Will medical tourism be the same as before? Consumers’ perspectives on health and wellness are changing. We need to view medical tourism more holistically. Medical wellness is set to be the next big thing where traditional practices and modern medicine coalesce creating unique wellness experiences. Medical wellness is aimed at the holistic enhancement of health and well-being with a broader perspective of effective prevention, intervention, and preservation. The primary goal is not only the treatment and healing but the motivation and guidance to maintain sustainable well-being throughout life.

Why medical wellness?

Short-term well-being

long-term

For decades, people have focused too much on treatment and medication overlooking the basics of well-being. • Chronic stress is linked to six leading causes of death namely cardiovascular diseases (CVD), lung disorders, cancer, liver cirrhosis, accident, and suicide. • 7 out of 10 deaths worldwide are caused by NCDs (noncommunicable diseases) like hypertension, obesity, diabetes, cancer, and respiratory ailments. All these by and large are preventable.

Medical tourism is picking up but at a cautious pace. Even though the pandemic is waning, economic uncertainties and geopolitical tensions have put brakes on the usual growth process. Prospective medical tourists are looking for holistic solutions in medical tourism that will fulfill their immediate medical needs as well as give the roadmap for life-long management of health and well-being.

• Cardiovascular diseases account for a majority of NCDrelated deaths.

Traveling is not as easy as before. Patients no longer would prefer the hurried get-the-treatment-and-fly-back schedule. They will be willing for a worthwhile investment to avoid long-term suffering in the future.

• More than 75% of insurers expect an acceleration of healthcare costs in the next 3 years.

• Global healthcare spending could cross $10 Trillion by 2022. • Many countries are unable to bear the expense load of NCD treatments.

Medical travelers are generally long stayers than conventional tourists. Post-pandemic, this duration is likely to increase. On average, they are most likely to stay for a minimum of 14 days often crossing a month. They are likely to spend the recuperation phase in some exotic wellness resorts amid various relaxing and rejuvenating activities. A medical tourist after undergoing heart surgery or joint replacement has to take up different rehabilitation programs and check-up sessions. What if both the treatment and recuperation period can be under the same roof arrangement with a well-synchronized service? This is the main objective of medical wellness ensuring a better and more consistent quality of medical and associated wellness services making the entire venture a delightful experience for patients.

Medical Wellness – Best Practices Despite the fundamental differences between medical tourism and wellness tourism, integrated medical wellness is seen in renowned medical facilities, health resorts, sanatoriums, and wellness resorts. Their service platform is focused on holistic medical wellness with equal emphasis on preventive and curative programs with the right combination of western medicine and eastern holistic healing practices. To build a successful medical wellness setup, the following are key considerations:

Feasibility It is also important to understand the feasibility of wellness integration from the existing positioning of the facility. Wellness treatments should be selected carefully ensuring their outcome is in sync with the medical treatments that will fulfill the goals of a patient’s holistic wellness journey.

Create the right organizational structure In an integrated medical wellness structure, exclusive medical and wellness-focused teams must work in sync each aware of their goals and responsibilities. To sustain a seamless patient flow, both teams should be led by respective medical and wellness directors. The top leadership to monitor that the medical and wellness teams under two directors are cohesive and deliver an exceptional patient experience.

Getting the right experts To integrate medical wellness, it is essential to hire licensed and certified wellness therapists. Experts should display utmost professionalism to create a great ambience of safety, trust, and comfort. Any lapse here will impact the synchronized operation and the patient’s objectives.

Proper equipment management Integrating advanced wellness treatments like Cryotherapy, 44 October 2022  w w w. me d g a te to d a y.co m

Hydrotherapy, Hyperbaric Oxygen therapy, etc. may demand an exclusive setup with certified technicians, nurses, or assistants. Have a proper system in place to run the entire operation successfully.

Building strategic partnerships If the facility does not have adequate space or resources for the necessary integration, partnerships with established wellness resorts or facilities can serve the purpose. Such partnerships can help in expanding medical wellness offerings without additional fixed costs.

DR. PREM JAGYASI

Brand positioning An integrated service platform will attract a broad customer segment. A dedicated sales and marketing team for each customer segment is vital to position your brand as a medical wellness destination. This helps in a well-defined promotion of medical wellness very specific to a group. Merging of medical and wellness will evolve as a widespread practice but this needs careful planning. Any mishap can ruin the image of a destination forever. To avoid this, it is always recommended to hire a reputed medical wellness consultant and advisor to guide facilities towards a successful venture.

India is at the cusp of medical wellness confluence India commands a formidable position in the global medical tourism and wellness tourism market. The country with its state-of-the-art medical facilities, excellent talent pool, and age-old traditional healing practices is best positioned to tap the emerging medical wellness trend. What Kind of Biohacking Will Win in the Long Run? SAJEEV NAIR

Founder & Chairman Vieroots Wellness Solutions

S

ajeev Nair is a pioneering biohacker in India who is credited with bringing geno-metabolic-based personalized epigenetic lifestyle modifications into the country. Mr. Nair, through his bestselling biohacking book ‘The Making of a Superhuman’, proved the efficacy & safety of bringing together modern medical research and biohacks from ancient systems like Ayurveda, Yoga & Meditation. Mr. Nair is the co-founder of Vieroots Wellness Solutions, a health-tech startup focusing on practical and safe biohacking; and Limoverse, the world’s first blockchain-based health & wellness ecosystem and metaverse connecting wellness seekers with wellness experts.

46 October 2022  w w w. me d g a te to d a y.co m

COVER STORY

Biohacking is one of those emerging ideas that mean different things to different people. On one hand, it can range from do-it-yourself biology using garage labs to self-health using various means. And in self- health too, it can range from adopting research-validated lifestyle modifications in diet, exercise, sleep, meditation etc, to the grinder subculture of implanting chips, injecting chemicals, undergoing geneediting using CRISPR and following cyborg-like ambitions.

relatively faster growing developing economies like India & China. Another major thrust for the wellness and biohacking industry is coming from the rapid deployment of technology. For example, the wellness genomics market which is the backbone of personalized wellness is poised to touch $12.8 billion, whereas its other enabler, the digital health records market is projected to reach $48.2 billion by 2025 itself.

The adoption of biohacking by the healthcare industry is closely linked with the growing size of the wellness sub-sector within the overall healthcare sector. This is because all the varied forms of biohacks with an aim of better self-health are all about ensuring wellness or the lack of diseases, rather than treating diseases. Biohacking gels perfectly with wellness as they both concern themselves with not only prevention of diseases, but also bettering physical and mental performance.

There are several other drivers too for the fast growth of wellness and biohacking. In fact one of the greatest drivers for this market is the wearables and quantified self tools, which is growing at a rapid rate to touch $118 billion by the end of this decade. While the original devices were smartwatches, fitness bands, pedometers & smartphone apps, today this segment has unbelievable innovations like pocket EKGs that can detect all the common cardiac arrhythmias.

Wellness vs Treatments

What Kind of Biohacking Will See Faster Adoption?

Currently, 10-20% of the global healthcare industry is made up of the wellness sub-sector which is dominated by nutritional supplements, specialized diets, fitness programs, biohacking and such lifestyle industries. With the healthcare industry recently crossing the $10 trillion dollar mark, this means the wellness sub-sector is already valued between $1-2 trillion now. But growing forward, wellness is poised to do much better than the overall healthcare sector.

This is not a million dollar or billion dollar question anymore, but a multi-trillion dollar question. Everyone from hospital systems to pharma giants to tech majors to private equity leaders to giant mutual funds to the best stock market analysts are desperately trying to crack this puzzle - which way will the wellness and biohacking industry swing finally?

This is because wellness is growing at a faster clip of 15% as against healthcare’s overall growth rate of 7%. According to a major study by a global consultancy major, this will result in around 60% of the global healthcare industry consisting of prevention or wellness activities by 2040, overtaking even the classic reactionary model of treating diseases. There are several reasons for this enormous growth awaiting the wellness sub-sector.

The Role of Biohacking in Wellness On an average, around 10% of the GDP of developed nations like in North America & Europe are coming from their health & wellness industries, whereas in developing nations like India it is close to low single digits. However, in recent years, the USA has clearly diverged from this trend with healthcare & wellness accounting for 18% of its GDP. This in turn is driven by several trends in this largest healthcare & wellness market in the world. For one, lifestyle modifications which are the most proven biohacks have gone mainstream in the US after the health insurance industry started covering them much like they provide for treatments like medicines, procedures & surgeries. Sooner or later, this trend is sure to spread to other developed nations and even to

Three pointers are enough to answer this invaluable question. Firstly, what will win eventually is what will deliver quantifiable good health & wellness. On this count, genomics based personalized lifestyle modifications is the clear winner. This is the gold standard in wellness and biohacking today as such modifications are epigenetic which means capable of overriding even genetic risks for diseases. Secondly, what will win is those biohacking tools that can be mass adopted. On this count, the wearable wellness & biohacking devices is an obvious winner. Health & wellness ecosystems and metaverses that bring in gamification and rewards using such wearables are also expected to boom. On the other hand, grinder technologies like implantables, injectables & gene editors have a clear disadvantage as there is great resistance for their mass adoption. Thirdly, the safety of the biohacks will be a major concern deciding the eventual winner. On this count, the clear winner is likely to be the time-tested and totally safe biohacks from ancient wellness systems like Ayurveda & Yoga. Indeed, in the words of Dave Asprey, widely considered as the Father of Biohacking, “We have been biohacking forever. Ayurveda is an ancient biohacking system to put yourself in better control of your own biology.”

INTEGRATED CARDIOLOGY SOLUTIONS CAN BE PANACEA FOR INDIA’S CARDIOVASCULAR DISEASE BURDEN

I

ndia suffers from a huge cardiovascular disease (CVDs) burden with one in four deaths (2.5 million/ year) due to CVDs including ischemic heart diseases. The Global Burden of Disease study states that the age-standardised CVD death rate is 272 per 100,000 population in India, much higher than that of the global average of 235. India had approximately 55 million CVD cases in 2016 and just before the pandemic, there were only six lakh angioplasties in the country each year. Despite such a huge disease burden, there are around only ~2000 active Cath Labs available in about ~140 cities in the country. To tackle such a massive disease burden, there is an opportunity for healthtech companies to provide innovative, integrated cardiology solutions at each point in a patient’s journey—and connect them across the care pathway to improve care and quality of life for patients. These innovations have the potential to help provide enhanced clinical insights, strengthen clinical confidence, drive efficiency in critical care and improve cardiac care experiences from diagnosis and treatment to home care, delivering better experiences for clinicians, patients, and staff. A powerful ecosystem for cardiovascular care comprises of software-defined systems supported by Artificial Intelligence (AI) and connects care along the entire patient journey (from emergency care, diagnosis, treatment, post-acute to at home care) with uniquely integrated imaging, devices, software, informatics, and services. This ecosystem can deliver datadriven insights to provide the right care at the right time to help better manage even the most complex cardiology cases. Smart integrated end-to-end technology streamlines communication between devices and systems to help identify and address clinical as well as

Image Guided Therapy, Philips Indian Subcontinent operational improvements, strengthen clinical confidence, build workflow efficiency, optimize resources, and help lower the cost of care. There is scope for transformation of cardiac care delivery in India into a connected future by integrating imaging, devices, software, informatics, and services including: Minimally invasive treatment Minimally invasive/ image guided therapy (IGT) procedures have seen a major uptake in the recent times. These procedures are completely safe (lessinvasive/short-recovery) and provide better clinical outcomes in most selected patient groups. A large part of this success can be attributed to the way digitally sophisticated products such as cath labs empower experienced physicians with clear images. Further, AI enabled smart applications on these products helps the operator tackle several disease challenges. A cath-lab is a good example where a physician can tackle multiple cardiac issues under fluoroscopy, which, traditionally would have been addressed by complex surgeries. With the rise of IGT procedures in India, cardiac interventions such as angioplasties

have become routine. Over six hundred and fifty thousand angioplasties are performed annually in India and are growing at a high single digit rate each year. Futuristic cath-labs, also referred to as interventional suites, combine multiple advanced technologies to yield varied solutions than the traditional outputs. This approach allows catheterbased interventional therapies to expand their capabilities and reach broader patient groups with higher success rates. It also enables multiple specialty doctors to make the best decisions for their patients while protecting themselves and their patients from avoidable radiation exposure during X-Ray procedures. With 1000+ active cath labs in India and growing strongly, we continue to demonstrate our commitment to address the burden of cardiovascular disease in the country. However, we clearly acknowledge a huge unmet clinical need owing to the high CVD disease burden and have aggressive plans to expand our cath labs in the next several years to reach to the remotest of locations, helping the medical community tackle more complex patient procedures.
