I am not well educated and cannot understand English at all hence I took the help of a local agent to fill my online application form. 2. During filling the form, Agent had asked me “In last 5 years have you been convicted by a court in India for any offence and sentenced to imprisonment for more than two years?” 3. I informed to the agent about the CRIME NUMBER 386/2017 SECTION 323.504 IPC AT PS CIVIL LINE DATIA. I also informed to him that this case is now closed and I have not received any imprisonment in this case. 4. Based on the above disclosure by me the agent thought that there is no need to disclose this information hence this information was not reflected in my passport application. I would like to clearly state that I had not hided this information intentionally. This information could not get

disclosed due to the mine and agent’s inability to clearly understand the requirements of passport application form. Self-attested photocopies of the documents related to the case are attached.
