Mutual Termination Agreement A Mutual Termination Agreement (MTA) is used to terminate a contract by mutual consent of both parties. Learning Objectives What Is the Purpose of An MTA? MTA Overview Below is an example of an MTA, the interactive markers highlight key aspects of an MTA and how Order Management reviews an MTA for the booking and/or cancellation process. a contract can't be performed and/or a customer ended relations.     


Quote Number The quote number referenced on an MTA is the legally binding contract that PowerSchool has with the customer. In this example, the customer is in contract per Q-645294 and is requesting to terminate that contract. 





Termination The cancellation effective date is based on the quote start date or the first day of the renewal the customer does not wish to renew. It is important to look at the Customer Asset 2.0 to verify the cancellation effective date as the quote start date on the PDF copy can differ from the booking date 

S-Mart has a flexible paid time off (PTO) policy. Employees can use their accrued PTO for vacation, personal time, volunteering, or other activities. Guidelines Lesson 5 of 9 moniquegarcia Paid Time Off Relaxing and getting away can help lower anxiety and blood pressure, which lowers the risk of illness caused by stress.


Exceptions If you have any questions or concerns about our PTO policy, please contact your supervisor or the Human Resources department. Each full-time employee will accrue PTO biweekly in hourly increments based on their length of service. PTO is added to the employee's PTO bank when paychecks are issued. PTO taken will be subtracted from the employee's accrued time bank in one-hour increments. Contract employees, temporary employees, and interns are not eligible to accrue PTO. PTO is not earned in pay periods during which unpaid leave, disability leave, or workers' compensation leave are taken. Employees who miss more than three consecutive unscheduled days may be required to present a doctor's note. PTO accrued before the start of an approved unpaid leave of absence must be used to cover hours missed before the start of the unpaid leave. PTO taken in excess of the PTO accrued will be unpaid.  Employees are paid for the PTO they have accrued at the end of the employment period. If an employee has used PTO time not yet accrued and employment is terminated, the PTO taken will be deducted from the final paycheck.


S-Mart pays 70% of the premiums, and you pay the remaining 30% of premiums. If you work more than 30 hours per week, you're eligible for insurance benefits on the first of the month following your start date. We have a designated medical insurance provider. You'll receive your medical card 10-15 days after enrollment. Contributions Employee-only medical coverage: $90.33 Employee-partner medical coverage: $189.68 Employee-child(ren) medical coverage: $158.07 Employee-family medical coverage: $275.48 We have a designated dental insurance provider. To get your dental card, contact Human Resources. Vision insurance is provided through a designated insurance company and uses a specific network. Contact Human Resources to get your vision card. Group Plans S-Mart's income protection plan provides a financial cushion in case of your disability or death. S-Mart pays 100% of the premium. To receive this tax-free disability benefit, you will be taxed on the value of the insurance. Lesson 7 of 9 moniquegarcia Life and Disability Insurance We offer life, accidental death, and disability insurance to give you peace of mind. Life Insurance Annual salary with a minimum of $25,000 coverage and a maximum of $450,000.


Voluntary Plans All full-time S-Mart employees are eligible to purchase voluntary life insurance at discounted group rates. Contact Human Resources for more information. Accidental Death and Disability Insurance Annual salary with a minimum of $25,000 coverage and a maximum of $450,000. Disability Insurance 60% of monthly earnings to a maximum of $6,000.


S-Mart's payroll policy describes the payroll process. Documented procedures ensure a clear and defined approval process, efficient payroll activities, availability of forms, and appropriate controls. S-Mart's payroll policy establishes internal checks and balances to control and protect one of our largest expenses. Payroll Information Pay periods are every two weeks. All paychecks for two-week pay periods will be distributed within five working days after the last working day of the pay period. Federal and state regulations require employers to maintain accurate employee time records. Nonexempt employees are responsible for recording all time worked accurately in the timekeeping system. S-Mart currently utilizes two methods of time recording for employees. The method used is determined by location and/or position. PAY PERIO DS TIM EKEEPIN G O VERTIM E PAY


Overtime may be necessary to handle emergencies, heavy workloads, or other issues. Record overtime hours accurately and consistently. Advise employees to work overtime only when they have to finish urgent work. Have all necessary provisions to allow employees to complete their work during standard working hours. Payroll System You can access our payroll system to do the following: Access your pay stubs Review and update demographic information (address, personal email, phone numbers, emergency contact information, etc.) Enroll in our benefits platform Review the employee directory
