Dear Mr. Buijs, We refer to the Program Grant Agreement between The Global Fund to Fight AIDS, Tuberculosis and Malaria (the "Global Fund") and the CARE International Ecuador (the "Principal Recipient") for a tuberculosis program entitled "Expansion of DOTS Strategy in Ecuador", effective 29 September 2005 and amended by letters dated 19 July 2006, 11 May 2007, 18 December 2007, 24 January 2008 and 14 February 2008 and a Phase 2 Amendment effective 7 April 2008 (as amended from time to time, the "Grant Agreement"). Capitalized terms used but not defined in this letter have the meaning given to them in the Grant Agreement. By signing this letter, the Global Fund and the Principal Recipient agree to amend the Grant Agreement by: (1) attaching to Annex A of the Grant Agreement the document entitled "Performance Framework 3a: Indicators, Targets, and Periods Covered" attached to this letter; and (2) amending block 4A of the face sheet of the Grant Agreement as follows: Block 4A: Modification Number: 6 (Implementation Letter 6 dated 4 July 2008). Achievement of the indicators set out in the Performance Framework 3a to Annex A of the Grant Agreement, in addition to other relevant information, shall be used by the Global Fund as a basis for determining disbursements of Grant funds. The face sheet of the Grant Agreement, as amended, is attached. Except as modified herein, the Grant Agreement remains in full force and effect. This amendment shall come into force on the date of signature of this letter by both parties. Please confirm your agreement with the foregoing by signing both enclosed copies of this letter and returning one to us, retaining one copy for your records.
