Adult Day Care Adult Day Care Programs are designed to offer a safe environment for functionally impaired older adults who are unable to remain home alone. These services allow seniors to stay in the community as long as possible, while providing caregivers respite on a regular basis. Adult Day Care Programs also give older adults an opportunity to socialize with others and remain as independent as possible. Two main types of adult day programs have developed over the years: adult day services and adult day health services. Adult Day Care Services (Social) The services provided in this setting focus more on the social aspect of the individual. Most programs are open five days a week and some provide services on the weekends. Transportation to these programs may be provided or arranged by the organization if not otherwise available through the family. An array of services are typically provided at an adult day center including: nursing care, social work, occupational, physical, and recreational therapy, meals (at least one hot meal is typically provided), transportation, personal care, educational programs, counseling and supervision, and crafts. Adult Day Health Services (Medical) These services are similar to the adult day services, but they also provide health care services to meet the needs of the clients. These services are provided to adults with physical and mental disabilities, while offering respite to their caregivers. The goals of these programs are to restore and maintain the individual’s capacity to care for

Types of Payment Most people pay privately, but there are other means of payment as well. Many facilities are certified for Medicare and Medicaid, but each center differs on the coverage available. Some agencies offer scholarships or payment on a sliding scale, which means that the cost is adjusted to the needs of the client. Since each agency differs, the best way to find out about payment for each separate agency is to call.

Long-Term Care Ombudsman A Long-Term Care Ombudsman is an advocate for residents of nursing homes, board and care homes, and assisted living facilities. Ombudsmen provide information about how to find a facility and what to do to get quality care. They are trained to resolve problems. If you want, the ombudsman can assist you with complaints. However, unless you give the ombudsman permission to share your concerns, these matters are kept confidential. Under the federal Older Americans Act, every state is required to have an Ombudsman Program that addresses complaints and advocates for improvements in the long term care system.

VA operates the nation’s largest integrated health care system with more than 1,400 sites of care, including hospitals, community clinics, nursing homes, domiciliaries, readjustment counseling centers, and various other facilities.
Eligibility for most VA benefits is based upon discharge

Aid and Attendance (A&A) is a benefit paid in addition to monthly pension. This benefit may not be paid without eligibility to pension. A veteran may be eligible for A&A when: 1. The veteran requires the aid of another person in order to perform personal functions required in everyday living, such as bathing, feeding, dressing, attending to the wants of nature, adjusting prosthetic devices, or protecting himself/ herself from the hazards of his/her daily environment, OR, 2. The veteran is bedridden, in that his/her disability or disabilities requires that he/she remain in bed apart from any prescribed course of convalescence or treatment, OR, 3. The veteran is a patient in a nursing home due to mental or physical incapacity, OR, 4. The veteran is blind, or so nearly blind as to have corrected visual acuity of 5/200 or less, in both eyes, or concentric contraction of the visual field to 5 degrees or less. Housebound is paid in addition to monthly pension. Like A&A, Housebound benefits may not be paid without eligibility to pension. A veteran may be eligible for Housebound benefits when: 1. The veteran has a single permanent disability evaluated as 100-percent disabling AND, due to such disability, he/she is permanently and substantially confined to his/her immediate premises, OR, 2. The veteran has a single permanent disability evaluated as 100-percent disabling AND, another disability, or disabilities, evaluated as 60 percent or more disabling. A veteran cannot receive both Aid and Attendance and Housebound benefits at the same time. How to Apply for Aid and Attendance and Housebound: • You may apply for Aid and Attendance or Housebound benefits by writing to the VA regional office having jurisdiction of the claim. That would be the office where you filed a claim for pension benefits. If the regional office of jurisdiction is not known, you may file the request with any VA regional office. • You should include copies of any evidence, preferably a report from an attending physician validating the need for Aid and Attendance or Housebound type care.

What is Hearing Loss? Hearing loss is a common problem caused by noise, aging, disease, and heredity. Hearing is a complex sense involving both the ear’s ability to detect sounds and the brain’s ability to interpret those sounds, including the sounds of speech. Factors that determine how much hearing loss will negatively affect a person’s quality of life include: • The degree of the hearing loss • The pattern of hearing loss across different frequencies (pitches) • Whether one or both ears is affected • The areas of the auditory system that are not working normally—such as the middle ear, inner ear, neural pathways, or brain • The ability to recognize speech sounds • The history of exposures to loud noise and environmental or drug-related toxins that are harmful to hearing • Age Hearing loss is one of the most common conditions affecting older adults. Approximately 17 percent, or 36 million, of American adults report some degree of hearing loss. There is a strong relationship between age and reported hearing loss: 18 percent of American adults 45-64 years old, 30 percent of adults 65-74 years old, and 47 percent of adults 75 years old, or older, have
a hearing impairment. Men are more likely to experience hearing loss than women. People with hearing loss may find it hard to have a conversation with friends and family. They may also have trouble understanding a doctor’s advice, responding to warnings, and hearing doorbells and alarms. Hearing loss comes in many forms. It can range from a mild loss in which a person misses certain high-pitched sounds, such as the voices of women and children, to a total loss of hearing. It can be hereditary or it can result from disease, trauma, certain medications, or long-term exposure to loud noises. Hearing problems that are ignored or untreated can get worse. If you have a hearing problem, you can get help. See your doctor. Hearing aids, special training, certain medicines, and surgery are some of the choices that can help people with hearing problems. Care Option Choices When considering a care community, older and disabled adults are the primary consumers of both assisted living and nursing facility care. They should be the primary decision makers, when possible, about the move from independence to an environment where they receive help in one or more areas of their daily life. The “resident” is involved in all aspects of the decision making process, health and financial assessment, identifying and touring potential
facilities, and considering charges in the contract/resident agreement. Surrogate decision makers can participate, but nothing is more important than involving the older adult in this process! There are two basic questions consumers should ask themselves when considering a change: 1. What are your needs? 2. How will you pay to have those needs met?
Consider These Things When Answering Your Basic Questions: Discuss the alternatives with family and friends.
• care by friends or family • home care or home health care agencies • shared living • independent apartment with services such as meals • assisted living • nursing facility care

Think about how you would handle possible crises. • Who would you want to make decisions for you if you become incapacitated?
• Have you designated someone to be your power of attorney? • Do you want to complete a living will or other advance directives about your medical care (Health Care Power of Attorney)?
Consider your financial resources.
• What resources are available to you - pension, savings, long-term care insurance, Medicare, Medicaid, Social Security, etc.? • What type of care (if any) is covered by your insurance? • How will the monthly fees and any deposits be covered?
Investigate the options you choose. • Use on-line resources such as the Center for Medicare and Medicaid services to see up to date information about home care, home health and nursing homes. www.cms.gov • Ask the assisted living for their most current corrective action reports, or their deficiency statements. • Visit the facilities you are interested in as often as needed to get a clear picture of what care, treatment and services are being offered. • Talk with present residents and their families about what life is like within the community or facility.
Read the contract carefully. Ask a trusted friend to review it with you. You may also want to discuss the contract with an attorney. • Request a copy of the contract prior to admission to review the language and prepare questions.
• Keep a copy of the signed contract on hand. • Regulated facilities are obligated to make sure that residents and their representatives understand the contract, the programs available to cover the services, and the limits of these programs.

Assisted living centers provide an environment that takes care of laundry, utilities, housekeep ing, maintenance, and activities while also providing staff to help with medication management, bathing, dressing, mobility, and other major issues that may arise in a person’s life. The goal: to keep people up and moving and independent in as many areas of life as possible while still giving people a leaning post when they need it.
STEPS FOR SELECTING AN ASSISTED LIVING FACILITY
Assisted Living
There comes a time in everyone’s life when the day-today task of taking medications becomes a little cloudy. At some point, we all will struggle with getting our buttons right on our shirt or keeping our balance while we try to pull our pants up to our hips. When that time does arrive, take comfort in the fact that there are options for help. Assisted living communities provide care for the individual who needs assistance with some of the activities found in the
The following steps may help you find a suitable home. If you are assisting a loved one in looking for a facility, include him or her in as much of the selection process as possible. Consider that person’s interests, ideas and wishes as you look at the options.
1. Ask friends, relatives, clergy and physicians about their experiences in particular facilities.
2. Contact the Ombudsman for information on types of adult care homes, location and services available and for public records information.

3. Contact the County Department of Social Services: • Financial aid. Applications for financial assistance can be made at the county Department of Social Services. A physician for all residents entering an adult care home or family care home must complete a condensed medical summary. • Talk with the Adult Home Specialist at the local Department of Social Services, which monitors the homes. Ask for an opportunity to review Monitoring Report Forms and/or Corrective Action Reports. These reports are public record. Visit the home. In addition to arranging a meeting with the administrator and/or supervisor-in-charge, make unexpected visits on weekends (during visiting hours) and at meal times. Identify yourself to the staff and ask if you might walk through the facility to determine if you or your relative could live there comfortably.

• Remember to respect the residents’ right to privacy as you visit. Use your eyes, ears, and nose to determine whether residents are receiving reasonable and proper care. • Ask about the administrator and/or supervisor's involvement with the facility and its residents the hours they are on site, the staff ratio, the stability of the staff and the manner in which problems are resolved.

5. Carefully examine the facility’s contract. Note what services are included and which services may require additional fees. If possible, have an attorney review the language of the contract to protect the interests of the resident. Check for language that asks consumers to waive their rights such as mediation clauses. 6. Review the facilities written policies. Are you comfortable with the requirements and guidelines? Smoking, for example, may not be allowed indoors or be limited to certain rooms. 7. Ask about any special needs the resident might have. Determine provisions made for people allergic to tobacco smoke or require a special diet. Determine that there is adequately trained staff to meet the resident’s need.

Assisted Living
All N.C. Residents entering an adult care home must have a basic health assessment within the first few days of admission. This N.C. regulatory assessment form is not used to set rates for reimbursement. However, some facilities set their “rate” of reimbursement on their own company assessment. There is NO STANDARDIZED formula for rate setting in North Carolina or for the forms used by facilities to determine private pay rates. Before signing a contract, consumers should clearly understand: • What aspects of resident care are being assessed and who is doing the assessment? What are their credentials? Does the consumer have an opportunity to participate or challenge the results? • Will the assessment create a change in how service is delivered? • What staff person(s) are responsible for the service? • What duration and frequency will the service be provided? • What staff person will be supervising the services? Since there is no rate setting standard in North Carolina, the costs of adult care homes can vary from $1,800 – $5,500 a month. Many facilities charge a basic fee for a complete package of room, board, and services. Others have a base fee and then charge from a “menu” of cost associated with each specific service. There may be other fees associated with a stay at the assisted living: • Entry or application fee

• Fees associated to admission to the secure unit designed specifically for persons with Alzheimer’s Disease or other types of dementia • Room holds when a resident is away for an extended period to the hospital or other health care setting. • Private room fees • Medication administration or packaging fee
• Activity fees • Special dietary needs may require extra fees for food or preparation Remember, that ALL charges for services should be specifically outlined in the contract/resident agreement and agreed to by both the resident and the facility. Many companies will negotiate special rates, discounts and other benefits to moving into their community. Be sure that these are also detailed in the agreement including the special rate, the end date of the “special” and if other promises are made about deals for the future, that these are also well documented and agreed upon by both parties. This might include having a room when the person changes their pay status from private pay to SCSA. Some long-term care insurance policies will pay for assisted living care. Verify this coverage before the move-in date so there is no surprise at the first month billing. The lease agreement or admission contract is also called the RESIDENT AGREEMENT. This important document indicates costs and responsibilities of the resident and/or their representatives to ensure payments to the facility in exchange for services. These are not regulated or standardized either. Consumers should: • Request a copy of the contract prior to admission to review the language and prepare questions. • Keep a copy of the signed contract on hand. • Facilities are obligated to make sure that residents and their representatives understand their rights, the language of the contract, the programs available to cover services and the limits of these programs.
• Leave of absence fees for packaging medication
• Has legal authority to handle the resident’s financial affairs.
• A pre-determined time to be a private pay resident

• An additional payment to supplement the cost for private rooms or rooms in a special care unit when a person receives State County Special Assistance

If a resident has been declared legally incompetent, a guardian or Power of Attorney may have to sign all of the necessary papers for admittance. BEWARE mediation clauses that may ask you to waive your right to contact an attorney!

SC Housing Search Usually managed by a city or county housing authority, these subsidized complexes can be found in many urban areas. These may also be privately-owned and exchange housing to low-income adults for government credits or subsidies. Some complexes are open to people of all ages but do reserve spaces for senior adults. Eligibility for this kind of public housing is based on income. The senior must be below a set limit in income and these income levels change yearly based on the median income in the given area. Subsidized housing rent is based on a percentage of the senior’s income, so rates vary for each individual. Affordable housing is often the same rate for all residents if the living unit is the same size (regardless of income). The Fair Housing Act (FHAct) protects all citizens from discrimination on the basis of race, color, national origin, religion, sex, handicap or familial status (families with children under the age of 18 living with parents or legal custodians; pregnant women and people securing custody of children under 18). Although the FHAct was amended in 1988 to prohibit discrimination on the basis of disability and familial status, Congress intended to preserve housing specifically designed to meet the needs of older persons. Such housing that meets the FHAct definition of “housing for older persons” is exempt from the law’s familial status requirements, provided that: HUD has determined that the dwelling is specifically designed for and occupied by elderly persons under a Federal, State or local government program or it is occupied solely by persons who are 62 or older or it houses at least one person who is 55 or older in at least 80 percent of the occupied units, and adheres to a policy that demonstrates intent to house persons who are 55 or older. Therefore, housing that satisfies the legal definition of senior housing or housing for older persons described above, can legally exclude families with children. Home Health
The term home health agency often indicates that a home care provider is Medicare certified. A Medicare-certified agency has met federal minimum requirements for patient care and management and therefore can provide Medicare and Medicaid home health services. Individuals requiring skilled home care services usually receive their care from a home health agency. Due to regulatory requirements, services provided by these agencies are highly supervised and controlled. Some agencies deliver a variety of home care services through physicians, nurses, therapists, social workers, homemakers and HCAs, durable medical equipment and supply dealers, and volunteers. Other home health agencies limit their services to nursing and one or two other specialties. Personnel are assigned according to the needs of each patient. Home health agencies assume liability for all care. When services are covered by Medicare and/or Medicaid, home care providers must bill their fees directly to the payor to Medicare or Medicaid. Providers often will bill other third-party payors directly as well. Any uncovered costs are later billed to the client. Medicare, Medicaid, and most private insurance plans pay for services that home health agencies deliver. Payment from these sources depends on whether the care is medically necessary and the individual meets specific coverage criteria. Individuals may opt to pay out of pocket for services that are not covered by other sources. There’s no place like home! Sometimes, nursing homes or retirement centers seem to be the only options for care. Nonmedical home care provider services allow people to maintain the independence of their daily routines and allow individuals to age gracefully in familiar surroundings. These providers accomplish this by sending loving and experienced caregivers that have been thoroughly screened and drug tested to care for your loved ones in their home with the ability to select your own caregiver. Services may include: loving companionship, meal preparation and diet monitoring, bathing or showering, grooming and dressing, incontinent care, hygiene assistance, medication reminders, recreational activities, transportation services, light housekeeping, changing linens, errands and shopping, respite for family caregivers, phone call checks and many other activities. Services can be provided on a temporary or long term basis up to 24 hours a day. Providers receive reimbursement privately, through Long Term Care Insurance policies or through community programs. Overall services and cost structures vary from provider to provider.

Anyone can initially refer a patient to hospice – physician, nurse, social worker, clergy, family, friends, or the patient. Information regarding the patient’s diagnosis and prognosis will be requested of the attending physician. Hospice will arrange a meeting with patient and family to discuss hospice care and services. There is no charge for consultation visits and the patient is not obligated to accept services. Early referrals are encouraged. Hospice care allows patient to maintain greater consistency in symptom management and live each day to the best of their ability.

Hospice care is designed to provide support to you and your loved ones during the final phase of life. Hospice care focuses on your comfort and provides a better quality of life, with the goal to enable you to have an alert, pain-free life and live each day as fully as possible. A person is eligible for hospice care when their life expectancy is six months or less, when they are no longer seeking aggressive treatment, and when their personal physician believes that hospice is the appropriate type of care. Patients with a terminal illness do not usually have to pay for hospice care. Currently, most hospice patients have their costs covered by Medicare, through the Medicare Hospice Benefit. Medicaid provides benefits that are very similar to the Medicare Hospice Benefits. The hospice philosophy accepts death as the final stage of life: it affirms life and neither hastens nor postpones death. Hospice care treats the person rather than the disease, working to manage symptoms so that a person’s last days may be spent with dignity and quality, surrounded by their loved ones. Hospice care is also familycentered – it includes the patient and the family in making decisions. Hospice care is used when you can no longer be helped by curative treatment and are expected to live about 6 months or less if the illness runs its usual course. Hospice gives you supportive or palliative care, which is treatment to help relieve symptoms, but not cure the disease. Its main purpose is to improve your quality of life. You, your family members, and your doctor decide when hospice care should begin. While a patient with any end-stage disease may be eligible for hospice care, the most common diagnoses seen are cancer, heart and lung disease, stroke, and end-stage Alzheimer’s. Getting into

A rehabilitation hospital provides care for stabilized patients who still need inpatient hospital care. Unlike the patient in the standard hospital, the patient sent to a rehab hospital needs additional help to recover from an injury or surgical procedure. They might require physical, occupational or speech therapy as their condition improves, and they might need social work assistance to determine how to live life once they are released. Like a regular medical hospital, a rehabilitation hospital has 24 hour nursing care, 7 days per week. Many nurses are RNs and rehab nurses. Rehab hospitals are required to use a coordinated interdisciplinary team approach led by a rehab physician, and includes a rehab nurse, a case manager, and a licensed therapist from each therapy discipline who must meet weekly to evaluate/discuss each patient’s case. All patients, regardless of diagnoses/ condition, must demonstrate need and receive at least 3 hours of daily therapy or 15 hours over a 7 day period. Patient must need 2 different therapies minimally (Occupational, Physical and/or speech therapy) and need medical supervision by a doctor. For the family of the patient, a rehabilitation hospital can be a positive intermediary step before a family member returns home. The rehab hospital not only instructs the patients and helps them regain function but also works with the family to determine what will be needed when the patient is released. Most rehabilitation hospitals offer comprehensive inpatient and outpatient rehabilitation services. For individuals recovering from a brain injury, spinal cord injury, stroke or other neurological impairment, the outpatient treatment program can be a very important step on the pathway to recovery. Patients receive intensive rehabilitation and are able to return home at night to practice newly learned skills in a familiar environment. The outpatient program helps prepare patients for living independently and returning to work.

Rehabilitation Outpatient Therapy Rehabilitation Outpatient Therapy
Outpatient Therapy If you’re recovering from an injury, surgery, or illness, you may be advised to consider outpatient therapy. Outpatient therapy is a course of treatment to support your recovery and rehabilitation, delivered through a series of visits to a private therapy practice or a clinic. It is also highly recommended for improving strength, mobility and balance for pre-surgery and everyday living. Unlike inpatient treatment, with an outpatient therapy program, you will stay in your own home and visit your treatment professionals over a series of weeks or months. Outpatient therapy programs can vary, but will usually include physical therapy and occupational therapy. In some cases, speech therapy might also be recommended. Some outpatient therapy programs also include a course of treatment with a counselor, nutritionist, or other rehabilitation professionals. Typically, your outpatient therapy program will begin with

an overall assessment of your treatment needs. You will then receive a treatment plan which will include a schedule of regular appointments with the rehabilitation professionals who best suit your needs. Your appointments might be once every one to five days, and will usually last between 30 minutes to an hour. You may be scheduled to see more than one therapist. Outpatient therapy treatment will also include exercises and activities that you will be

expected to complete at home between appointments. The focus of outpatient therapy is to help you regain your normal function. Rehabilitation specialists will design a program which includes services like physical, occupational, or speech therapies which are customized to your unique medical needs, lifestyle, and requirements, to help you get back into the swing

Today’s senior centers are evolving to reflect a new view of aging that empowers the people they serve. They’re connecting older adults with meaningful work and volunteer opportunities, and increasing their access to valuable benefits and resources. Through evidenced-based program, they’re helping them manage their health and finances so they can continue to live in their homes as long as possible. From career counseling and financial planning to tools for staying healthy, there is no doubt senior centers work! Senior centers are designated as community focal points through the Older Americans Act. The National Institute of Senior Centers defines a senior center as a place where “older adults come together for services and activities that reflect their experience and skills, respond to their diverse needs and interests, enhance their dignity, support their independence, and encourage their involvement in and with the center and the community.” Not only do senior centers offer helpful resources to older adults, they serve the entire community with information on aging; support for family caregivers, training professionals, lay leaders and students; and developments of innovative approaches to addressing aging issues. While senior centers typically provide nutrition, recreation, social and educational services, and comprehensive information and referral, many centers are adding new programs such as fitness activities and Internet training to meet the needs and interests of the new generation of seniors.

Transportation Assistance Transportation Assistance Did you know that one-half of Americans 65 and older do not have access to transportation? And that more than half of all non-drivers 65 and older stay at home in a given day because they don’t have transportation options. Those in rural areas and small towns are particularly affected because the transportation options are limited. But it’s important for seniors to remain mobile to keep their social independence with friends and family; to reduce feelings of isolation and loneliness, and many other life prolonging benefits. If you or your loved one is no longer able to drive, there are personal transportation options through state programs, nonprofits and private businesses.
