The 2023 Benefit Enrollment Begins Immediately Following 90 Days Of Employment

Eligibility Requirements (subject to waiting period) • Full-time employees (30 hours per week) • Dependents of full-time employees including spouse and/or dependent children to age 26

An eligible employee may join the plan at open enrollment or within 30 days of a qualifying event.

We began in 1998 as a family-owned and operated business. At Brysan, we thoroughly love the ability to serve our community in any way that we can. Ask a Hygienist : Dental members can simply email their dental questions to a team of licensed dental professionals who in turn will respond in about 24 hours. Dental Health Risk Assessment : We want our dental members to better understand their oral health and their risk factors for tooth decay, hum disease and oral cancer. This easy to use online tool can help them do this. Dental Care Cost Estimator : In order to help our dental member better understand the cost of their dental care, we offer access to a user-friendly, webbased tool that provides estimates on common dental procedures and treatments when using a network dentist. More Capabilities : With our latest mobile application, members can find a network dentist as well as view heir claims. Our application is available for both Android and Apple phones.

Dentists In Your Plan Network

You'll save money when you visit a dentist in your plan network because Anthem and the dentist have agreed on pricing for covered services. Dentists who are not in your plan network have not agreed to pricing, and may bill you for the difference between what Anthem pays them and what the dentist usually charges To find a dentist by name or location, go to anthem.com or call dental customer service at the number listed on the back of your ID card. Lets Talk Supplemental Benefits Employee Voluntary Life + Spouse Voluntary Life

All Employees can elect up to the Guaranteed Issue amount of $70,000 without having to complete a medical questionnaire. This is a "one-time only" availability. Employees can elect up to the Guaranteed Issue amount of $25,000 for their spouses without having to complete a health medical questionnaire. This will be the only time this is offered. This coverage is based on your (the employee) age. Spouse coverage cannot exceed 50% of the Employee's amount. If you select more than 50%, the amount will be bumped down to the next closest amount that is 50% or below. Spouse coverage terminates when you (the employee) reach the age of 70. All Late Entrants (ex. enrolling in 2024's open enrollment) will be required to complete a medical questionnaire and coverage is not guaranteed.

Employer Paid Coverage

Basic Life Insurance coverage, pays benefits to your beneficiary(ies) if you die from most causes while coverage is in effect.

Accidental Death and Dismemberment (AD&D) insurance coverage adds low-cost accidental death protection by paying benefits in the event that your death is due to an accidental cause.

Short Term Disability

All employees are eligible to enroll in Short Term Disability without completing a medical questionnaire.

Any Late Entrants (ex. 2024's Open Enrollment), Employees will be required to complete a medical questionnaire and coverage is not guaranteed.

Accident Insurance

Accident coverage provides a cash benefit in one lump sum if you or a covered family member is injured because of an accident. Use accident coverage to help pay for out-of-pocket medical costs, such as ambulance fees, physical therapy, X-rays or daily expenses like rent, food, transportation. This plan covers accidents that occur both at and outside of the workplace. Key Features: No cash benefit is paid directly to you in lump-sum, tax free payment No medical questions or exam needed to enroll You can take your coverage with you even if you leave your employer No limitations or pre-existing conditions

Critical Illness Insurance

Critical Illness coverage provides the added layer of security you want and need when illness occurs— a lumpsum cash benefit to help pay for unexpected costs. You decide how to use the benefits to best support recovery for yourself or a family member. Use your critical illness coverage to help pay for out-of-pocket medical costs, such as for prescriptions, hospital bills, Xrays or daily expenses like rent, food or transportation. Key Features : Cash benefit is paid directly to you in a lump-sum, tax free payment $50 payment towards health screenings, such as lipid panel or fasting glucose test. You can take your coverage with you even if you leave your employer. Convenience We are here to help. To file a claim, start with the claim form provided by your employer. Hospital Indemnity Plan

Hospital Indemnity provides a lump-sum, tax-free cash benefit to help pay for costs that can come with a hospital stay that your health plan doesn’t cover. Use your hospital indemnity coverage to help pay for out-ofpocket medical costs or daily expenses like rent, food or transportation. Key Features : Cash benefit is paid directly to you in a lump-sum, tax free payment Covers hospitalization for maternity from day one with no waiting period. You can take your coverage with you even if you leave your employer for up to three years. No limitations for pre-existing conditions. Convenience We are here to help. Glossary



Your deductible is the amount you pay out of pocket before your insurance provider pays a portion of the bill. There may be certain benefits, such as office visits and prescriptions, where the deductible does not apply. After you meet your deductible, coinsurance is when you and your insurance provider share the costs of your medical care. You pay a percentage and your insurance provider pays the remaining amount until you hit your out-of-pocket maximum. A copay is a fixed payment you pay for a covered service. For example, you may pay a $35 copay to see your primary care physician. This is the maximum you pay out of pocket for medical care within a year. Any charges above this amount are paid at 100% by your insurance provider for the rest of the plan year. Deductibles, coinsurance, and copays are all included in our plans’ out-of-pocket maximums. The cost of your benefits is deducted from your paycheck before taxes are calculated and you are therefore only taxed on your remaining paycheck balance. You pay less taxes this way.

Benefit FAQ's 1 . What benefits am I currently enrolled in? a. To view your benefits selections for the active plan year, visit the Employee Self Service Center in ADP.

2 . How do I find out if my doctor is in network? a. Go to www.anthem.com and click on “Find Care”

b. If you are not yet registered and logged in under yourself, scroll down and select “Guests”.

c. Scroll down and follow these steps to help you with your search: i. Select what type of care you need or are searching for from medical, Dental, or Vision in that dropdown.

i. In the “What State do you want to search in” field, select Georgia as the plan is written in Georgia, even though it will cover your home area. ii. In the next drop down, “What type of plan do you want to search with?” select the following based on the type of plan you are searching for a provider in: 1. Medical: Select “Medical (Employer Sponsored)” 2. Dental: Select “Dental” 3. Vision: Select “Vision” iii. In the “select a plan/network”, select: 1. Medical – “Blue Open Access POS (Select Network)” if you reside in Georgia or “National Blue Card PPO” if you reside outside of Georgia 2. Dental – “Dental Complete” 3. Vision – “Blue View Vision” b. Select Continue c. You can search by provider OR run a general search

3. What medical plan should I choose? a. To learn more about the available medical plans, please review the Benefits Enrollment Guide. We also recommend that you use the “Plan Comparison Feature within the ADP Benefits Enrollment site if you are uncertain about which medical plan to choose for a side by side comparison of primary plan features or to see what benefits are available for the dental and vision plans.

4. Do I need to provide documentation (i.e., marriage/birth certificates) during benefits open enrollment? a. Not during open enrollment. b. If you are enrolling a Common Law Spouse as a qualifying life event, you will need to complete an Affidavit of Common Law Marriage. c. In some cases, you may be eligible for mid-year change through a Qualifying Life Event (QLE) – such as a birth/adoption, marriage, or divorce. In this case, documentation will be required to process your change. 5.What happens if I need medical care outside of my home coverage area? a. All individuals enrolled in an Anthem medical plan are covered for urgent and emergency care anywhere in the world. “Covered” means that expenses will apply towards your deductibles and out-of-pocket maximums, and insurance will cover the claim as if it were within the network.

6. Are routine services covered outside of the home coverage area? a. If you are needing urgent or emergency services and are able to, contact your insurance company before seeking care outside of your home coverage area. Using the SYDNEY mobile app is the best way to look up in network providers.

7. I have a child who is away at college and needs medical coverage. What do I do? a. Children under 26 years of age may be covered under their parents’ medical, dental, and vision insurance plans. Anthem offers plans/benefits for dependents who live outside of their home coverage area. Check to see if Anthem has physician and medical; offices in the area where your child will be living. You can do this through the SYDNEY mobile App.
