- Mostly people didn’t wear safety glass (Not comply Life Saving Rules (LSR 11) agreement number 2)
Investigation:
- People complaint have foam at safety glass when do the work because of face mask. - Safety glass missing
Action:
- Give awareness (Toolbox Safety Briefing) safety glass and why we need wear and effect when ignore LSR 11. - Inform to logistic pass new safety glass to supervisor and pass to their teammates.
