List of Worldwide Collection Locations Drug and alcohol testing services including benzene biomonitoring The following document lists ports and other locations worldwide where sample collections may be carried out by Alere Toxicology Collecting Officers. This is not a comprehensive list. For ports and locations not in this document please contact your representative, or the Alere Toxicology Medscreen Sales Department (toxeu.international@ alere.com). Please be aware that the locations listed in this document can be subject to change at short notice. Our fully trained international collection network provides valuable support for your Medscreen drug and alcohol testing service as well as benzene testing programmes. In most cases our collectors reside in, or close to, the locations listed and all costs of collection are included in the overall per–sample pricing indicated in quotations or service agreements. In some circumstances, extra costs are incurred by Alere Toxicology when arranging or carrying out collections, even in standard listed ports. These may include, but are not limited to, the following: nn Launch charges for vessel visits. nn Port entry administration charges. nn Travel, accommodation and subsistence charges for attendance at unlisted ports. nn Travel, accommodation and subsistence charges incurred due to circumstances beyond Alere Toxicology’s control. nn Site–specific requirements for protective clothing or equipment. In such cases, charges will only be made at the cost as incurred by Alere Toxicology, with no administration charges payable. Please note that some ports and locations require at least 48 hours notice for collections. Benzene collections In addition to the self–collection kit available from Medscreen a full collection service is available in a number of the listed ports worldwide. As Alere Toxicology can provide Benzene testing at a restricted number of ports, in some instances two weeks’ notice may be required. Please contact your regional representative, or the Medscreen Sales Department ([email protected]), in order to discuss your specific requirements.

Alere Toxicology Offers You Two Programme Management Options: 1. Random programme management Alere Toxicology can manage your random testing programme. You will receive a certificate confirming that you are registered with Alere Toxicology and a further certificate when your programme is completed each year. nn Tell us the number of vessels and how often you want them tested. nn Keep us updated with the vessel positions and agents’ details. nn Give us any special instructions about times of boarding etc.

2. Requested programme management Alere Toxicology will respond to your requests for testing: nn We need at least 48 hours notice for some of the ports on this list. nn Provide us with the agents’ details. nn Give us any special instructions about times of boarding. For both random and requested programmes, after each collection Alere Toxicology will leave a certificate on the vessel confirming that the collection has taken place and you will be notified when the specimens have arrived at our laboratory.
