SPRING ALLERGIES Each year, seasonal allergies contribute to 2,000,000 school absences and have a significant impact on students’ academic, physical, social, and emotional health. Seasonal allergies, hay fever or allergic rhinitis make people miserable, affecting up to 40% of children. School nurses play an important role in helping to keep kids who suffer from seasonal allergies thriving in school, despite the pollen counts. AVOIDING TRIGGERS

TREATMENTS

Taking steps to limit exposures to potential allergy triggers is one of the best ways to manage spring allergies. Providing these tips to your school community allows families to take proactive measures to keep their kids healthy during allergy season.

When a student suffers from allergy symptoms, it is important to refer them to their primary care provider to appropriately identify and confirm specific allergens and determine the best treatment plan. Providers may even recommend starting medications before peak pollen seasons even begin. There are a number of prescription and over the counter treatments available that can alleviate symptoms including:

• Keep windows in the home and car closed • Wash hands and face after being outside • Shower or bathe before bed • Use air conditioning in the home and car • Vacuum frequently

• oral antihistamines and decongestants • topical eye drops • nasal sprays

• Wash bedding weekly • Encase padded finishings like mattresses in allergen-proof, zip-up covers • Check pollen counts and plan outdoor activities accordingly

100 YEARS OF ADHESIVE BANDAGES In 1920, Josephine Knight Dickson complained to her husband, Earle, about the frequent nicks, burns, and cuts she was suffering while cooking and working in their home. At the time, the only real option for covering a small wound was to use separate gauze or cotton balls and adhesive tape. Each “bandage” would have to be personally cut and sized, inevitably falling off within an hour or two of application. The lack of options for hygienic wound coverings combined with no availability of antibiotics yet at the time meant infection posed a serious risk. Fortunately for Knight Dickson, Earle worked for Johnson & Johnson as a cotton buyer and immediately began brainstorming what sort of supplies he could use to make a simple, sanitary covering for his wife’s injuries that would stay firmly in place as she carried on through her daily activities. Bringing home antiseptic cotton gauze and surgical adhesive tape that was included in Johnson & Johnson’s suite of sterile surgical products, Dickson took a piece of the gauze and attached it to the center of a piece of the tape, then covered it with crinoline to keep it from sticking to itself. From there, he rolled the tape into a ready-to-go spool that his wife would simply have to unfold and snip to the desired size. Both Josephine and Earle were so impressed with the ease and efficacy of this innovative wound-covering solution that Earle decided to share the idea with his boss at Johnson & Johnson. Not long afterward, a small test batch of what the company called, BAND-AID Brand adhesive bandages were manufactured in exactly the way Earle had originally made them, in a roll that had to be cut with scissors. As with many new and cutting-edge products, educating people on how to use the newfangled wound coverings proved challenging, and sales at first were painfully slow. Johnson & Johnson remained committed to the Band-Aids and deployed traveling salesmen to teach pharmacists and physicians how to properly use

them. In 1924, new machinery allowed the company to mass produce and create the pre-cut, individually wrapped bandages we now know as Band-Aids. The success of Band-Aids was so monumental and the product so ubiquitous, the name became synonymous with all other adhesive bandages. But since its invention, the global medical tapes and adhesive bandages market has grown to an estimated USD 9.35 billion in 2022 and many exceptional brands are making adhesive bandages while continuing to enhance wound care and innovate bandage technology. In the late 1990s, a company called Ebon-Aides launched the first adhesive bandages designed for people of color. For the first time, there was a bandage that could be considered “flesh-colored” for a diverse range of skin tones and now, thankfully, there are many brands that are inclusive of all races and ethnicities. Additionally, materials and adhesives have evolved to provide various levels of durability, breathability, and adhesion while a wide range of sizes and shapes offer wound care options specific to certain body parts including fingertips, knuckles, and knees. New bandage technology continues to emerge with the invention of bandages that clot just like blood (QuikClot) and “smart” bandages that give individuals an easy way to track their healing progress and signal infection concerns are being heavily researched. The evolution of wound care and adhesive bandages has significantly improved in the 100 years since Earle Dickson first invented a solution for his wife’s cuts and scrapes. At the same time, the role of the school nurse has, to quote a timeworn and trite news headline, “become more than just bandaids”. Adhesive bandages, nevertheless, remain a much-needed supply in schools, and the wide range of options now available provides a healing option for every student.

Assessment, Interventions, & Follow-up for Common Finger Injuries in the School Setting

Small Cuts or Scratches Kids’ hands are everywhere—sticky, dirty, poking, and playing, its no wonder a good percentage of them have a bandaid on at least one digit. For superficial cuts or scrapes, apply direct pressure with a clean, sterile gauze to stop any bleeding and wash thoroughly with soap and water for at least five minutes. If the wound still appears dirty, gently scrub to remove any remaining muck or grime. Administer an antibiotic ointment (when authorized) and cover completely with an adhesive bandage. The bandage should be changed daily and a tetanus shot (booster) may be necessary for certain lacerations so be sure to check immunization records and follow-up with the student’s parents to provide appropriate guidance.

Jammed Finger Dodgeball, kickball, basketball, you name it, the sports balls at school cause a lot of jammed fingers. This injury occurs when the end of a straightened finger or thumb receives a blow and the tip of the finger is compressed toward the hand. This results in the ligaments that support the finger joint being stretched and “sprained”. Careful evaluation is critical to assess finger position, range of motion, pain, and swelling. It is most helpful to compare the injured finger to the same finger on the other hand that is uninjured. Rest, ice, and elevation for any type of acute finger injury reduces pain and inflammation while soaking the hand in cold water for twenty minutes and then re-evaluating, can also prove valuable. If the pain is severe, range of motion is limited, or the finger is deformed, fracture must be ruled out. For short-term immobilization and support, “buddy-taping” the injured finger to an adjacent uninjured finger is effective.

Ice and elevation can help alleviate pain, while reducing inflammation and throbbing. Instructions should be provided to parents on changing the bandage daily, appropriate pain interventions, and monitoring for any signs of a subungual hematoma which may require further evaluation and draining.

Torn Nail Broken or torn fingernails, especially distal to the nail bed, aren’t typically significant injuries, although they can be very upsetting to students. For a cracked nail, if there are no uneven or jagged edges, simply leave it alone and provide reassurance to the student. If the nail is almost torn through and a large flap is present, sterile scissors should be used to cut along the edge of the tear to prevent it from catching on anything, potentially causing further damage. Additional interventions for torn nails include ice or soaking in cold water for 20 minutes, application of an antibiotic ointment (when authorized), and covering with an adhesive bandage, changed daily. It is important to communicate to the student and parent that it can take six to eight weeks for a new nail to grow in.

Crushed/Smashed Fingertip Student fingers seem to have an uncanny ability to get slammed in car doors and school lockers. While fractures are not common with this type of injury, if one does occur, there is risk for bone infection. Most often, this will present with a blood blister or abrasion and sometimes, a damaged nail. Wash the area with soap and water for at least five minutes and if any cuts are present, apply an antibiotic ointment (when authorized) and cover with an adhesive bandage.

As an essential part of their role, school nurses collaborate closely with administration, public health officials, community providers, and faculty and staff to determine and uphold environmental infection control measures.

CLEAN DISINFECT SANITIZE Common Finger Injuries in the School Setting Cleaning uses soap (or detergent) and water to physically remove surface soil, germs, and other impurities from surfaces. Rather than killing germs, cleaning removes them, thereby reducing the number and lowering the risk of infection transmission.

Disinfecting is killing or inactivating germs on surfaces by using chemicals. Best practice is to disinfect surfaces after cleaning, to kill any germs that were not physically removed in the cleaning process, further lowering the risk of spreading infection.

Sanitizing reduces the number of pathogens on surfaces to a safe level, as outlined by public health standards or regulations. Sanitizing can be done by cleaning, disinfecting,or both. Sanitizing kills bacteria on surfaces but is not intended to kill viruses.

LISTEN UP! NOISE IN SCHOOLS In recent years, the number of kids and adolescents affected by noise-induced hearing loss (NIHL) has been steadily rising, even though it can oftentimes be an unrecognized problem. Studies suggest that one in every six to eight middle and high school students has detectable hearing loss likely due to extreme exposure to noise. School settings, whether in classrooms, hallways, or extracurricular activities, can be loud. According to a report by the World Health Organization (WHO), North American children “may receive more noise at school than workers from an 8-hour work day in a factory.” Repeated exposure to excessive noise can cause both temporary and permanent hearing loss, and noise pollution, even when below dangerous levels, can also lead to students being distracted, losing focus, and having difficulty concentrating, resulting in diminished learning. NIHL occurs when loud noises damage the hair cells in the inner ear, damaging the hearing nerve. When the nerve is injured in this way, it is called sensorineural hearing loss or nerve deafness. Many other things can cause sensorineural hearing loss, but exposure to extreme noise, especially when prolonged or repeated, can either suddenly or gradually lead to this type of hearing loss which is irreversible, once the nerve is destroyed. When an individual has noise-induced hearing loss, they may experience difficulty hearing higherpitched sounds, and words with “sh”, “s”, “th”, and “f” are often indistinguishable, and can sound like people are mumbling. Additionally, signs of NIHL include pain and discomfort, ringing/buzzing in the ears, and speaking loudly or shouting.

How much noise is too much? Routine exposure to noise that is 85 decibels or higher can cause hearing loss.

What can schools do? • Educate kids on healthy hearing habits including the three things they can do to protect their hearing: lower the volume, move away from noise that feels too loud, and wear hearing protection such as earplugs. • Inform parents on strategies to help protect their children’s hearing and provide information on volume-limiting headphones and setting volume limits on devices.

• Measure the noise levels (there are several free apps available) in different areas of the schools including gymnasiums, music classes, hallways, and the cafeteria. Work with school administration, faculty, and staff on reducing noise levels in the loudest areas. • Train staff on safe hearing practices including recognizing potential signs of hearing loss in students. • Stock and supply earplugs to be used at school during noisy assemblies, concerts, or sporting events. • Follow state-mandated regulations for conducting hearing screenings using evidence-based best practices.

What does this sound like? A normal conversation is usually about 60 decibels while jet planes or sirens are about 120–130 decibels. When listening to music through headphones at the highest volume level, data suggests noise typically measures at or above 110 decibels.

THE RIGHT TEMPERATURE School nurses are more than familiar with students visiting the health office with a tactile fever, as determined by a concerned teacher, or as the story often goes with older children, “my friend said I felt hot.” Fevers are frequently one of the most common reasons students visit the school nurse and having a consistent method to measure temperatures is essential for the school nurse practice. A fever is typically defined as a temperature of 100.4 degrees Fahrenheit (38 degrees Celsius) and higher, however, ranges for what is considered a “normal” temperature vary depending on the route used to measure. Due to the variance between measuring methods, schools’ fever policies may define a fever to be either 100.4°F or anything above 101°F. A reliable thermometer for the school nurse is fundamental to performing accurate assessments and providing appropriate interventions, and understanding how the thermometer works, including proper technique, is important for the overall quality of care.

Axillary The axillary method is the least accurate measure of body temperature and readings can be 0.5°F to 1°F lower than oral temperature readings. The average axillary temperature is 97.5°F and many pediatric providers consider an axillary temperature of 99°F or above to be a fever since it more specifically measures peripheral body temperature instead of core. While considered a minimally invasive technique and commonly used in children, an axillary temperature measurement takes longer and can be affected by sweat and improper probe placement.

Oral The average oral, or sublingual, temperature is 98.6°F and this route is generally considered accurate and appropriate for use with children five and older. The major drawback to using an oral thermometer in the school health office is the difficulty for kids to properly keep the thermometer in place under the tongue (in the posterior sublingual pocket on the side of the frenulum) and their inability to keep their mouths closed long enough for a valid reading. Additional variables include mouth breathing, eating/drinking, and exercise.

Tympanic Using infrared rays to measure the temperature inside the ear canal, tympanic thermometers are both fast and reliable. The average tympanic temperature is approximately 97.8°F and is considered a precise measurement of core body temperature because of the shared blood supply between the tympanic membrane and hypothalamus from the internal carotid artery. Relatively non-invasive and very well tolerated by both younger and older children, using a tympanic thermometer can be extremely convenient in the school setting, however, readings can be affected by improper positioning, atypical ear canals, cold temperature exposure, and the presence of cerumen or infection.

Temporal Blood moving through the temporal artery in the forehead allows a temporal thermometer to accurately measure body temperature through an infrared scanner. Temporal temperature readings tend to be about 1°F cooler than oral temperature readings with the average temporal temperature range being 95.4°F to 98.1°F. Preferred by many school nurses and other healthcare providers for their speed and completely non-invasive technique, it’s important to note that readings from these types of thermometers are very easily influenced by external factors including direct sunlight, indoor heat, drafts, sweat, and user technique. Thermometers are one of the most important tools in a school nurse’s toolkit. The right one will be dependable, consistent, and easy to use while meeting the needs of the school nurse’s workflow and supporting their efforts in keeping kids healthy.
