Dear Parents, Welcome to the Meeker Elementary School community! We are very excited you have chosen to attend our school! Thank you for entrusting us with the education of your child. We take this responsibility very seriously. Know we will do everything possible to help your child grow and learn at the highest possible levels. At Meeker we believe in creating a safe and stimulating learning environment for all of our children. Parent involvement and effective communication are highly valued at our school. We are continually working toward the goal of becoming a “community of learners.” We invite you to become involved in this very important mission. Our school handbook is designed to provide you with important information about our school community. Please review this document and keep it in a handy place for future reference. Home/School Communication COMMUNICATION Meeker is on a monthly format for our newsletter, which is posted on our website (meeker.greeleyschools.org). If you would like a hard copy sent home with your child, please contact the office at 970-348-2000.

Birthday Parties BIRTHDAYS - CELEBRATIONS -Private birthday or any other invitations are NOT to be passed out at school. -If balloons, flowers, etc. are delivered to Meeker for students, the teacher will be notified that the delivery is in the office and when it is convenient, the teacher will allow the student to come to the office to see their delivery, but it will remain in the office until the end of the school day. -Birthday treats may be brought to school, and are generally passed out during the last 5-10 minutes of the day. Please make arrangements for bringing treats with your child’s teacher. We ask that you adhere to the Wellness Policy as well as the Hep A Prevention guidelines below. The entire district Wellness Policy can be viewed at www.greeleyschools.org/studentwellness The district will support and promote proper dietary habits contributing to students’ health status and academic performance.  Encourage parents and staff to contribute healthful food and beverage items to classroom celebrations. The district will provide parents and teachers with an approved list of healthful, commercially prepared foods to contribute to classroom celebrations. To ensure that students and staff have access to healthy foods and beverages, it is the goal of the district that all foods listed should be healthy:  All foods/beverages available during school parties/celebrations will be healthful items Specific nutrition standards include:  Not more than 30% of the total calories of the food item, excluding nuts or seeds, is from fat.  Not more than 10% of the total calories of the food item, excluding reduced-fat cheeses, eggs and nut butters, is from saturated fat.  Not more than 35% of the total weight of the food item, excluding fruits or vegetables, is composed of sugar.

Hepatitis A Prevention Guidelines A. CLASSROOM FOOD PREPARATION 1. Before beginning any project involving food preparation, proper hand washing techniques are essential. 2. Anyone having any open sores on their hands should wear gloves for food preparation. 3. Food should be prepared and stored at proper temperatures. 4. Gloves should be worn whenever serving food. (Available in each classroom) B. FOOD ITEMS BROUGHT TO SCHOOL Gloves should be worn whenever food is served at school. Commercially prepared/packaged foods are encouraged. C. HEALTH EDUCATION The importance of GOOD HAND WASHING after using the toilet and before preparing and/or eating food, cannot be over-emphasized. D. SPECIAL EVENTS - If there are questions about special events scheduled at school, please consult with the School Nurse or the District Nurse Liaison.

Homework Policy HOMEWORK HAS MANY BENEFITS: 1. To communicate with parents about the curriculum taught in the classroom. 2. To practice skills taught. 3. To teach students how to manage time in the classroom and at home. 4. To foster responsibility. 5. To build independent study skills. 6. To help students make the connection that learning takes place anywhere. Parent Role: Parents should assume the role of facilitator. As students advance from grade to grade, the parent should allow for more independence in the completion of homework. Parents or siblings should never complete homework for the student as that student is not learning. If the homework assignment is too difficult for your child to complete mostly on their own, please have a conversation with your child’s classroom teacher. It is very helpful to provide a consistent time and quiet place for your child to complete homework, making sure they have all of the supplies needed. Finally, please make sure your child has completed their homework correctly.

Grade Level Student Expectations: Kindergarten: Kindergarten students at Meeker Elementary are assigned graded homework. Reinforcement activities for reading and math will be sent home on Mondays for the week. Parents are asked to read to or with their child each evening for 15 minutes. This could include bed-time stories, student take-home reading booklets, or 15 minutes of fun reading. Students should be exposed to as much new vocabulary as possible during the day and during evening reading. In addition parents are asked to work with their child on a literacy activity and a math activity each day for about 5 minutes.

First Grade: A total of 30 minutes of homework nightly, Monday through Thursday: 20 minutes of reading. Parents must sign the reading log each night. 10 minutes of math homework each night.

Second Grade: A total of 30 minutes of homework nightly, Monday through Thursday: 20 minutes for reading each night. 10 minutes for math, math facts, and/or spelling each night.

Third Grade: 20 minutes of nightly literacy practice (to include reading, vocabulary, spelling, and writing activities). 10 minutes of math practice (including math facts practice and homework completion).

Fourth Grade: 30 minutes of reading and completion of reading log 5 nights a week. 10 to 15 minutes of either math or spelling practice 4 nights each week, Monday – Thursday. Parents are expected to sign their child’s planner each evening.

Fifth Grade: Fifth grade students at Meeker Elementary will receive nightly homework to reinforce skills learned during the school day. Nightly assignments may include: 1. Finishing incomplete daily assignments 2. 20 minutes of nightly reading/writing 3. Test Preparation 4. 10 minutes of nightly math facts practice

Field Trips As stated on the Board of Education Policy IJOA, field trips are student excursions and/or experiences for the purpose of gaining firsthand knowledge away from the classroom. The teacher shall provide the parents with information concerning the purpose and destination of the trip, transportation and eating arrangements, date and time of departure, and the estimated time of return. A parental permission slip shall be required for each student participating in the trip, including walking excursions. One or more adults (school personnel and/or parents) may accompany the class on the field trip. Parents wishing to attend the field trip will be identified by a predetermined process, and teachers are responsible for informing accompanying adults of their duties and responsibilities. Parents may need to provide their own transportation and pay their own way. The teacher shall review acceptable standards of conduct with the students in advance of the trip. The teacher has primary responsibility for the conduct of the students. Students who cannot be self-controlled or teacher-controlled may be excluded from field trips.

Extended Day Programs 

Tech Club - Tech club is open to 4th and 5th grade students and is limited to 10 students each quarter. Students will incorporate technology by doing podcasts, a once a week vidcast (a video newscast), and other fun tech projects throughout the year! There will be a fee (yet to be determined) to participate.



Homework Club - First through Fifth graders meet in the Computer Lab Tuesday - Thursday from 7:45-8:15 and 3:45-4:15pm to get assistance with their homework from middle and high school students. Referrals for this service are made by classroom teachers.



Art Club - Art club is available for 3rd, 4th, and 5th grade students. Students will need a signed permission form to participate. Sign up with Mrs. Bjork to receive a permission form. We meet on Tuesdays after school from 3:45 to 4:30. Regular attendance is required for art club members.



Honor Choir-Choir is open any student in 3rd through 5th grade. We meet on Tuesday and Thursday mornings from 7:30-8:10.This group is for you if you love to sing, work hard, have treats, perform, and have fun! We will perform for the school and our parents various times throughout the year. Come join us!



Percussion/Ensemble - Percussion Ensemble is open to fifth grade students who are excited to be part of an outstanding musical group. This group meets on Wednesday and Friday morning from 7:30-8:10. We learn to play more complex music selections on the barred instruments, drums, and other percussion instruments. We will perform for the school and our parents during the winter concert.



Mountain Lion Club-Mountain Lion Club is a before school program that provides social skill development and practice. We meet Monday through Friday and is open to students in kindergarten through fifth grade.



Student Council- Student Council will be open to 3rd, 4th & 5th grade students. Meetings will be held afterschool, twice a month. Throughout the school year, student council members will demonstrate leadership by participating in community and school service projects, fundraising and promoting school pride!



Intramural- Intramurals will be open to 4th & 5th grade students. Intramurals will be held after school, three days a week and will run in 4 week sessions. Students will participate in a variety of physical activities with hopes to encourage lifelong fitness. Activities may include softball, flag football, jump rope, wall & rope climb, and basketball, fitness and dance.

Safety Physical safety Physical safety includes measures such as visitor check-in and identification badges, preparedness planning, and regular practice drills for a variety of potential incidents. Crisis plans for individual schools and the district as a whole are aligned with the recommendations of Homeland Security, local emergency responders, and the National Incident Management System (NIMS). Psychological safety Psychological safety provides for the social and emotional well-being of students. We works to maintain a positive school climate through bullying prevention, asset development, the Positive Behavior Support program, and early identification and intervention for at-risk students. Crisis management School District 6 has a crisis management plan that is reviewed on an annual basis. This comprehensive document provides a resource to school administrators in the event of a variety of crisis situations. Safe Schools Act compliance At the beginning of every school year, each school in our District is strategically evaluated and given a vulnerability assessment consistent with the Safe Schools Act. The key components of this audit include an evaluation of crisis response and disaster mitigation plans, the condition and safety of our individual facilities, the use of environmental design to prevent or disrupt crime, and the mechanism for dealing with bullying behaviors. Lockdowns and drills Each school in District 6 has a school lockdown plan specific to the individual school. Lockdown drills are frequently held to train staff and students in preparation for a potential actual emergency. Our schools also hold fire drills and tornado drills. Visitor management Our District has an active visitor management policy that controls entry into schools while also maintaining a welcoming atmosphere for parents and other visitors. Police collaboration Our District enjoys excellent collaboration with both the Greeley and Evans police departments. The Greeley Police Department has assigned school resource officers to our middle schools and high schools, and the Evans Police Department has assigned personnel to work with our elementary and K-8 schools located in Evans. Campus monitors Our District employs security monitors at our middle and high schools. These employees provide a visible presence with regard to student, staff and facility security. In addition, their assignment entails security peripheral to neighborhoods surrounding our schools. FERPA

Family Educational Rights and Privacy Act Weld County School District 6 schools maintain educational records on each student for the purpose of planning instructional programs, for guidance of students, for preparation of state and federal reports, and for research. These records are protected under the Family Educational Rights and Privacy Act (FERPA). Students have the right of privacy. The principal of each school shall be responsible to protect and secure from scrutiny all student education records, without written signed permission of the eligible/adult student or parent, except by school officials, or other individuals with a legitimate educational interest (i.e., administrators, teachers, counselors, support staff, school board members, contracted individuals, and representatives of district committees). Personally identifiable information may be disclosed, transferred, or released without prior consent of the parent, student or guardian in connection with enrollment in another school, in an emergency situation, application for financial aid, research, a state statute, accrediting organization, a lawfully issued subpoena, and parties to an interagency agreement among social services, school and law enforcement authorities, and other signatory agencies for the purpose of reducing juvenile crime. Educational records are maintained in accordance with state and federal policies and contain identifying data (student and parent name, address, birth date, sex, race), academic record, standardized test results, attendance records, and health data. The record may also contain family background information, verified reports of serious or recurrent behavior patterns, record of extracurricular participation and participation in special programs, psychological reports, and anecdotal records of professional staff. Parents, legal guardian, or eligible students have the right to inspect and review records, a right to a copy of the record for actual cost of copying, a right to contest information contained in the record, and right of waiver. Transfer, disclosure, or release of student records requires written consent of the parent or legal guardian or eligible student except as outlined in this document. A parent who wishes to review or request an amendment to his/her child’s record should make an appointment with the principal or school counselor. An appointment will be scheduled within forty-five (45) days of the request Whatever rights are vested in the parent or guardian shall pass to the student whenever the student has attained eighteen years of age or is attending an institution of post-secondary education. Parents shall have access to their dependent children’s records regardless of age. Weld County School District 6 reserves the right to release the following “directory information” without prior permission of the parent: The student’s name, address, telephone number, if it is a listed number, date and place of birth, major field of study, participation in officially recognized activities and sports, weight and height of members of athletic teams, dates of attendance, degrees and awards received, electronic mail, media coverage of school activities, and the most recent previous educational agency or institution attended by the student. Permitting disclosure of directory information allows the District to publish your student’s name and/or picture in yearbooks, school newspapers and rosters/programs in relation to athletic, musical or dramatic performances. Additionally, the District will allow your student to be photographed or interviewed by district staff and general news media to promote school or classroom activities. The parent may refuse to permit the designation of any or all of the above categories as directory information with respect to their child by notifying the principal in writing within ten (10) calendar days of this notice or within ten (10) calendar days from the enrollment date of the affected school year. Lists of students will not be released except by Board action. If Weld County School District 6 is not in compliance with this law, complaints may be filed with the Family Policy Compliance Office, U.S. Department of Education, Washington, D.C. 20202.

Under the No Child left Behind Act, we must meet federal rules related to teacher qualifications. These rules allow you to learn more about your child’s teachers’ training and credentials. We are very happy to provide this information to you. At any time you may ask: • Whether the Colorado Department of Education (CDE) has licensed or endorsed your student’s teacher for the grades and subjects taught. • Whether CDE has decided that your student’s teacher can teach in a classroom without being licensed or qualified under state regulations because of special circumstances. • The teacher’s college major; whether the teacher has any advanced degrees, and if so, the subject of the degrees. • Whether any teachers’ assistants or similar paraprofessionals provide services to your child and, if they do, their qualifications. Our staff is committed to helping your child develop the academic knowledge and critical thinking he/she needs to succeed in school and beyond. That commitment includes making sure that all of our teachers and paraprofessionals are highly skilled. If you would like to know any of the information mentioned above, you must complete a Teacher Request Form. Greeley-Evans School District 6 Statement of Intent to Comply with the Protection of Pupil Rights Amendment Notice to the Parents / Guardians of a Greeley-Evans School District 6 Student: To comply with federal legislation, specifically the Protection of Pupil Rights Amendment (PPRA), except as otherwise required by law, students in the Greeley-Evans School District 6 shall not be required to submit to a survey, analysis, or evaluation related to curriculum or other school activities that is intended to reveal information, whether the information is personally identifiable or not concerning the student or the student's parents/guardians, without written parental consent, that reveals information concerning: 1. political affiliations 2. mental or psychological problems potentially embarrassing to the student or the student’s family 3. sex behavior or attitudes 4. illegal, anti-social, self-incriminating and demeaning behavior 5. critical appraisals of other individuals with whom the student has a close family relationship 6. legally recognized privileged or analogous relationships, such as those with lawyers, physicians and ministers 7. income (other than that required by law to determine eligibility for participation in a program or for receiving financial assistance under such program) 8. religious practices, affiliations or beliefs of the student or the student’s parents/guardians School personnel responsible for administering any such survey, analysis or evaluation shall give written notice at least two weeks in advance to the student's parent/guardian or the eligible student (student 18 years old). Discipline Policy JK - Student Discipline File: JK

Student Discipline The Board believes that effective student discipline is a prerequisite for sound educational practice and productive learning. The objectives of disciplining any student must be to help the student develop a positive attitude toward self-discipline and socially acceptable behavior. All policies and procedures for handling general and major student discipline problems shall be designed to achieve these broad objectives. Dis-orderly students shall be dealt with in a manner which allows other students to learn in an atmosphere which is safe, conducive to the learning process and free from unnecessary disruptions. The Board in accordance with state law has adopted a written student conduct and discipline code based upon the principle that every student is expected to follow accepted rules of conduct and to show respect for and to obey persons in authority. The code also emphasizes that certain behavior, especially behavior that disrupts the classroom, is unacceptable and may result in disciplinary action. The code shall be enforced uniformly, fairly and consistently for all students. All Board-adopted policies and Board-approved regulations containing the letters “JK” in the file name constitute the discipline section of the legally required code. Immunity for enforcement of discipline code An act of a teacher or other employee shall not be considered child abuse if the act was performed in good faith and in compliance with Board policy and procedures. A teacher or any other person acting in good faith and in compliance with the discipline code adopted by the Board shall be immune from criminal prosecution or civil liability unless the person is acting willfully or wantonly. Remedial discipline plans The principal may develop a remedial discipline plan for any student who causes a material and substantial disruption in the classroom, on school grounds, in school vehicles or at school activities or events. The goal of the remedial discipline plan shall be to address the student’s disruptive behavior and educational needs while keeping the child in school. Discipline of habitually disruptive students Students who have caused a material and substantial disruption, on school grounds, in a school vehicle or at a school activity or sanctioned event three or more times during the course of a school year shall be declared habitually disruptive students. Any student enrolled in the district’s schools may be subject to being declared a habitually disruptive student. Declaration as a habitually disruptive student may result in the student’s suspension and/or expulsion in accordance with Board policy concerning student suspension, expulsions and other disciplinary interventions. How do I find out about Emergency Closings? Occasionally, winter weather in northern Colorado can cause severe enough conditions that we have to consider closing schools or delaying the start of schools. This decision could be made because of significant snowfall, dangerous road conditions, or extreme cold. Deciding whether to close or delay the start of schools is always done with the safety of students, parents and staff in mind. District staff will do everything they can to make a decision as early as possible to allow parents to make alternative plans. School Closure If schools are closed for the day because of weather, students will remain home. Generally, this also means that all other school activities, such as after-school clubs, sports, practices, etc. are also canceled for that day. The lost instructional time may need to be made up later in the year in order to meet the State of Colorado's minimum requirements for annual instructional time for students. Two Hour Delayed Start If schools will open on a two-hour delayed start, then students should arrive at school two hours later than their normal start times. A delayed start could be used if weather or road conditions are severe overnight but expected to improve in the morning. On a delayed-start day, school buses will run their routes two hours later than normal. Parents who drive their children to school should drop their children off two hours later than the normal time. Notification If schools are to be closed or put on a two-hour delayed start because of poor weather and road conditions, then that decision will be announced by these means: 

Nutrition District Letter Weld County School District 6 Nutrition Services Department is taking a proactive approach to offering healthier and fresher foods for the students in our community. Our ‘home-cooked’ meals are prepared fresh daily utilizing whole, natural ingredients. Eliminating processed foods and replacing them with items made ‘from scratch’ has allowed the department to significantly reduce the amount of sodium, preservatives and other artificial ingredients that are served to our students. In addition, the Nutrition Services Department continues to expand its Farm to School Program by purchasing fresh produce from local growers. This provides our students with the freshest fruits and vegetables possible, while also supporting the local community. Low Balance/Charging Policy If your student(s) meal balance begins to run low, nutrition staff will provide verbal reminders to students and utilize auto-call and lowbalance letters to communicate the need for additional funds to be placed on the meal account. However, from time to time a student’s meal account may have insufficient funds to cover the cost of a meal. Our policy in District 6 is to allow Elementary students to charge up to two (2) breakfast and two (2) lunch meals, Middle School students to charge one (1) breakfast and one (1) lunch and we extend no charging privileges for high school students. All meal charges are to be paid in full by the end of the school year. Meal Payments Payments to your student’s account may be made a number of ways:  Cash can be sent with your student to pay for each meal and/or be placed on their meal account.  Checks are accepted at all schools for meal accounts.  Credit/Debit cards are accepted online at www.greeleyschools.org/payonline Please allow 24 hours for payment to post to your students’ meal account. Free & Reduced Meal Benefits Your student(s) may qualify to receive their breakfast and lunch meals for free or at a reduced rate. Meal Benefit Applications are mailed to each district household approximately 30 days prior to the start of the new school year and are also available in the school office and cafeteria and online on the district’s website. Only one application per household is necessary. Application processing can take up to 10 business days with families responsible for all meal costs until the application is approved. Once approved, meal benefits are in effect for the entire school year and carry over for the first thirty (30) days into the next school year. Wellness Policy Research shows that children perform better in school when they eat healthy and are physically active. MEALS AT SCHOOL We have a “no share” policy in the Meeker cafeteria. This means that to maintain health practices and avoid other issues, students are not to share any food with their friends. This includes hot lunch or lunch brought from home. It is fine that students bring food from a restaurant, but not to share. We appreciate your help with this in explaining it to your children. We also have a “peanut free” table in the cafeteria available for students with peanut allergies. No student that has a lunch containing any peanut product will be allowed to eat at that table. Soda is not allowed in the cafeteria as a beverage at lunch. Breakfast - School breakfasts are $1.40 each. Students wishing to eat breakfast at school, may arrive at 8:00 a.m. and must remain in the cafeteria until 8:05 a.m. Adult breakfast is $1.80. Lunch - Hot lunches are available daily at Meeker to students in the first through fifth grades. Kindergarten also eats lunch at school on their extended days. The cost is $2.25 per day ($11.25 per week). Credits can be purchased before school each morning in the cafeteria. Charging lunches is strongly discouraged. If it is unavoidable, pre-arrangements should be made with the school lunchroom manager. Please encourage your child to check on the credits he/she has and to keep up-to-date. Milk may be purchased by students bringing sack lunches at 60 cents per carton. Please feel free to join your child for lunch! Adult lunch is $3.25. Parents are encouraged to each with their child at lunch time, however when eating in the cafeteria you and your child will have 2 choices. Eat at the Grade level table with your child and other students. Eat at a separate table with your child, but no other students will be allowed to sit with you. Crosswalks Please do not park in the crosswalks because we want to keep them free so that students and families can walk across unobstructed. If you are driving through the parking lot, please stop at each crosswalk before proceeding.

 The Meeker Parking Lot will be coned off Monday’s 8:00-2:30, Tuesday-Friday’s 8:00-3:55.  You must also have a “Meeker Parking Pass” to park in the parking lot. If you park in the parking lot this is ONLY to drop-off or pick-up your child. You may only park there for a short amount of time.  When parking in the parking lot DO NOT park two cars wide, so that we can keep traffic flowing.  Please do not pull along the circle drive by the building since this is reserved for buses and day care vans.

Getting Involved Visiting School Meeker parents are always welcome at Meeker School! To increase awareness, we require everyone to enter Meeker School through the front door and sign into our visitor’s kiosk computer and then pick up a visitor sticker. We appreciate all of the wonderful parent volunteers at Meeker and we ask that you please sign in and out, so that we can record all of our many volunteer hours. We feel confident that you can appreciate this procedure, as we strive to make Meeker School a safe place to be for all students, staff and parents! School Accountability Committee Our school has established a school accountability which consists of the school administration, at least one classroom teacher, at least three parents, a representative of our community business partner, and one person from our community. This committee is charged with making recommendations to the school principal on such decisions as spending money and school improvement planning. The team meets quarterly to discuss whether school leadership, personnel and infrastructure are advancing or impeding implementation of the school’s performance and improvement. District 6 has School Health Clerks who are the main care-givers in health offices in all district and charter schools. They perform first aid and care for ill children, administer medications, perform health screenings, manage immunization and health records. They are available to answer questions and are able to contact School Registered Nurse Consultant at all times. Health Clerks have taken a health clerk training course and have yearly CPR, First Aid, Medication Administration, and other trainings as needed for specific student health needs.



District 6 has School Registered Nurse Consultants who work as a team with health clerks, school administration and staff to provide physical, mental, and social support to help children learn. School Registered Nurse Consultants are trained with an RN-BSN and have obtained School Nurse licensure through CDE. School Nurses are responsible for preparing health care plans, documenting health histories, collaborating with special education team on educational plans, delegating to unlicensed personnel who provide care for students, assisting with health education units, consulting with outside health providers and agencies.

Illness/Injury, Emergency Information  Accidents/Illness/Injury – o Health office staff or other qualified personnel will administer first aid for any ill or injured student. If needed, parent/guardians will be contacted and in extreme cases, 911 may be called.  Emergency information – PLEASE KEEP EMERGENCY CONTACT INFORMATION UPDATED WITH THE FRONT OFFICE STAFF. We need to be able to contact you if your child is ill or injured. Medications  If your child must have medication of any type during school, including prescription or over-the-counter medicine (pills, syrups, cough drops, eye drops, creams, ointments, inhalers, injectable), you have three choices: o Parents/Guardians may come to school and give it to their child at the appropriate time. o Parents/Guardians and health care providers may complete and sign a physician authorization form. The medication must be in a pharmacy-labeled bottle or original packaging with instructions. 52

Parents/Guardians may discuss with their health care provider an alternative schedule so the medication can be given outside of school hours.  Medications of any sort (over the counter or prescription) are not to be in the possession of students, except for those with written authorization from their health care provider, their family and school registered nurse consultant.  If your child will need any medication or treatment at school, please obtain a copy of the physician authorization form from the health office or download from health services website http://www.greeleyschools.org/Page/8183 Health Questionnaire / Students with Health Concerns  Health questionnaires - It is extremely important that parents/guardians complete the annual health questionnaire and Emergency Information form.  Students with known health concerns – School Registered Nurse Consultants write Health Care Plans (HCP) for students that require specific procedures at school. All students who have diabetes, seizures, severe allergies, severe asthma or who have any medication (prescription or over the counter) in school should have a HCP. Other disorders may also have specific precautions which would require a HCP. PLEASE contact the school Health Clerk or School Nurse if you have concerns about your child’s health.  Students with Severe Asthma, Severe Allergies, Seizure Disorder or Diabetes – forms are available from health office or on health services website for physician completion http://www.greeleyschools.org/Page/8183 Immunizations  Immunizations are an important part of our children’s health care and Colorado Law requires that children going to school be vaccinated to prevent vaccine-preventable disease. Students are not permitted to attend school without meeting immunization requirements or having a signed personal, medical, or religious exemption.  If parents have concerns about immunizations and vaccine safety, visit www.ImmunizeForGood.com . The Colorado Immunization Program’s website is located at www.ColoradoImmunizations.com.  Schools work hard to ensure compliance with the immunization laws. Your help in providing updated immunization records at school registration and when your child receives additional vaccine(s) is greatly appreciated. Screenings  Hearing and vision screenings are conducted annually by health services staff including District Audiologist. The following grades are screened: Pre-K, K, 1, 2, 3, 5, 7, and 9, all new students, and students with special education needs – per the State of Colorado guidelines.  Glasses when prescribed by an eye care professional are extremely crucial to student success. Please make sure students wear them daily at school.  Students who do not pass vision screening will receive a “referral” for follow-up care. Students who do not pass hearing screening will continue to be monitored by school audiologist. Loaned Clothing  The health office has a limited supply of extra clothing for accidents. Please wash and return these clothes at your earliest convenience. Parent Health Resources  Health office staff are happy to assist parents in finding health information and access to health care. Student should be diarrhea free for 24 hours without medication before returning to school. Recommended if the student has symptoms in addition to the fever such as a rash, sore throat, vomiting, diarrhea, etc. Student should be fever free for 24 hours without medication before returning to school.

MANUAL DE ESTUDIANTE/PADRES Servicios de Salud - ¿Quiénes somos?  Para formas e información de los Servicios de Salud, por favor vea la página de internet Servicios de Salud del Distrito 6. http://www.greeleyschools.org/Page/8187 www.greeleyschools.org > departments > health services  El Distrito 6 tiene asistentes de salud escolar quienes son las persona principales del cuidado en las enfermerías de las escuelas en todo el distrito y en las escuelas opcionales/chárter. Ellas dan primeros auxilios y atención a los niños enfermos, administran medicamentos, desempeñan revisiones de salud, manejan los expedientes de vacunas y salud. Ellas están disponibles para contestar preguntas y pueden localizar a la enfermera consultora registrada de la escuela en todo momento. Las asistentes de salud han tomado un curso de entrenamiento para asistentes de enfermería y tienen entrenamientos anuales de CPR (según sus siglas en inglés de reanimación cardiopulmonar), primeros auxilios, administración de medicamentos y otros entrenamientos según sean necesarios, para las necesidades específicas de salud de los estudiantes.  El Distrito 6 tiene enfermeras consultoras registradas de las escuelas que trabajan en equipo con las asistentes de la enfermería escolar, administración y personal de la escuela para dar apoyo físico, mental y social y ayudar a los niños con su aprendizaje. Las enfermeras consultoras registradas de las escuelas están entrenadas con un RN-BSN (Enfermera Registrada – Diplomado en Ciencias de Enfermería, por sus siglas en inglés) y han obtenido una licencia de Enfermera Escolar a través de CDE (Departamento de Educación de Colorado, por sus siglas en inglés). Las enfermeras escolares son responsables de preparar los planes de atención médica, documentar los historiales de salud, colaborar con el equipo de educación especial sobre los planes educativos, delegar personal sin licencia que da atención a estudiantes, ayudan con las unidades de educación de salud, consultan con proveedores y atenciones de salud fuera de la escuela. Enfermedades/Heridas, Información de Emergencia  Accidentes/Enfermedades/Heridas – o El personal de la enfermería escolar u otra persona calificada administrará los primeros auxilios a cualquier estudiante enfermo o herido. Si es necesario, se localizará a los padres/tutores y en casos extremos, se llamaría al 911.  Información de Emergencia – POR FAVOR MANTENGAN ACTUALIZADA LA INFORMACIÓN DE CONTACTO EN LA OFICINA PRINCIPAL. Necesitamos poder comunicarnos con ustedes por si su niño se enferma o está herido. Medicamentos  Si su niño debe tener medicamentos de cualquier tipo durante la escuela, incluyendo medicinas recetadas o sin receta (pastillas, tónicos, caramelos para tos, gotas para los ojos, cremas, pomadas, inhaladores, inyecciones), usted tiene tres opciones: o Padres/Tutores pueden venir a la escuela y dárselo a su niño a la hora apropiada o Padres/Tutores y proveedores de atención médica pueden completar y firmar una forma de autorización del médico. El medicamento debe estar en un envase de la farmacia con su etiqueta de información o en su paquete original con las instrucciones. o Padres/Tutores pueden hablar con el médico sobre un horario alternativo para que el medicamento pueda darse fuera del horario escolar.  Los medicamentos de cualquier tipo (con y sin receta médica) no deben estar en posesión de los estudiantes, excepto para aquellos con la autorización escrita de su médico familiar, su familia y enfermera registrada de la escuela.  Si su niño va a necesitar algún medicamento o tratamiento en la escuela, por favor obtenga una copia de la forma para la autorización del médico que encuentra en la enfermería de la escuela o bájela de la página de internet en servicios de salud: http://www.greeleyschools.org/Page/8183 Cuestionario de Salud/Estudiantes con Preocupaciones de Salud  Cuestionarios de salud – es sumamente importante que los padres/tutores completen el cuestionario anual de salud y la forma de información de emergencia.

Estudiantes con problemas conocidas de salud– Las enfermeras registradas de la escuela escriben los planes de atención médica (HCP, por sus siglas en inglés) para estudiantes que requieren procedimientos específicos en la escuela.  Todos los estudiantes que tienen diabetes, ataques, alergias graves, asma severa o quien tiene algún medicamento (con o sin receta médica) en la escuela, debe tener un HCP (Plan de Atención Médica, por sus siglas en inglés). Otros trastornos también pudieran tener problemas específicos los cuales requieren un HCP. POR FAVOR, comuníquese con la asistente de la enfermería escolar o la enfermera de la escuela si usted tiene alguna preocupación sobre la salud de su niño.  Para los estudiantes con asma severa, alergias graves, trastornos de ataques o diabetes – las formas están disponibles en la oficina de salud o en la página de internet de servicios de salud para que el médico lo complete http://www.greeleyschools.org/Page/8183

Vacunas  Las vacunas son una parte importante del cuidado de salud de nuestros niños y la ley de Colorado requiere que los niños que van a la escuela sean vacunados para prevenir enfermedades que se previenen por medio de las vacunas. Los estudiantes no tienen permitido asistir a la escuela sin cumplir los requisitos de vacunas o sin tener un permiso firmado por el médico o una exención religiosa.  Si los padres tienen preocupaciones sobre las vacunas y la seguridad de las vacunas, visite el sitio www.ImmunizeForGood.com . La página de internet del Programa de Vacunación de Colorado está localizada en www.ColoradoImmunizations.com.  Las escuelas hacen un esfuerzo por asegurar el cumplimiento de las leyes de vacunación. La ayuda de usted al proveer registros de vacunación actualizadas en la inscripción escolar y cuando su niño recibe vacunas adicionales es muy apreciado Revisiones  Las revisiones auditivas y de la vista son conducidas anualmente por el personal de servicios de salud incluyendo a la audióloga del distrito. Los siguiente grados son revisados: Pre-K, K, 1, 2, 3, 5, 7 y 9, todos los estudiantes nuevos y estudiantes con necesidades de educación especial – según las directrices del estado de Colorado..  Los lentes, cuando son recetados por un oftalmólogo profesional es sumamente crucial para el éxito del estudiante. Por favor, asegúrese que los estudiantes usen diariamente sus lentes en la escuela.  Los estudiantes que no pasen la revisión de la vista recibirán una “referencia” para darle seguimiento de atención de salud. Los estudiantes que no pasen la revisión auditiva continuarán siendo supervisados por la audióloga escolar. Ropa prestada  La enfermería escolar tiene un número limitado de ropa extra para accidentes. Por favor lave y regrese esta ropa lo más pronto posible para usted. Recursos de Salud para Padres  El personal de la enfermería escolar está dispuesto a ayudar a los padres a encontrar información de salud y de cómo tener acceso al cuidado de salud. Bienestar escolar  La investigación muestra que los niños se desempeñan mejor en la escuela cuando comen saludablemente y están activos físicamente. Para apoyar el rendimiento académico y una vida saludable, el Distrito 6 ofrece oportunidades interesantes de bienestar para los estudiantes y empleados. Recomendado para estudiantes que experimentan una tos severa, incontrolable o respirar con dificultad o resollar. Recomendado para estudiantes con otros síntomas además de diarrea, como: vomito, dolor abdominal, fiebre, la diarrea no puede ser contenido en un inodoro, hay sangre o mucosidad en la deposición. Estudiante no deberá tener diarrea por 24 horas sin medicamento antes de regresar a la escuela. Recomendado si el estudiante tiene síntomas además de la fiebre como una erupción temporal cutánea/salpullido, dolor de garganta, vómito, diarrea, etc. El estudiante no debe tener fiebre por 24 horas sin medicamento antes de regresar a la escuela. Recomendado si el estudiante está babeando incontrolablemente.

Recomendado si el estudiante tiene síntomas además del salpullido como cambio en comportamiento, fiebre, dolor de coyunturas, moretones no asociados con alguna herida o si la llaga está abierta o supurando. Recomendado si el dolor es severo, si el dolor aparece después de una herida o si el estudiante tuvo síntomas además del dolor estomacal como vómito, fiebre, diarrea, etc. Recomendado si el estudiante tiene síntomas además de las glándulas inflamadas como dificultad en respirar o tragar, fiebre, etc. Generalmente recomendado si el estudiante ha vomitado más de 2 veces en 24 horas, si el vómito es verde o con sangre, si el estudiante ha tenido una herida a la cabeza reciente o si el estudiante tiene síntomas además del vómito, como fiebre, diarrea, dolor estomacal, etc. El estudiante no debe tener vómito por 24 horas sin medicamento antes de regresar a la escuela Exclusión no es necesaria Exclusión no es necesaria. Comunicarse con la enfermera si el dolor de cabeza es persistente, severa, es acompañado de otros síntomas o si el estudiante tiene un plan del cuidado de salud Exclusión solo para niños a los que se les encontró con piojos vivos en la cabeza. La información no apoyan la exclusión escolar por liendres (larva/huevecillos de piojos) Los estudiantes con antibióticos por enfermedades contagiosas, como estreptococo, amigdalitis, bronquitis, conjuntivitis o neumonía, deben permanecer en casa hasta que el antibiótico haya sido ingerido por 24 horas. Toda lesión de piel debe estar cubierta por una venda o ropa durante el día escolar. Esto pudiera incluir impétigo, tiña/culebrilla cutánea, sarna y otra infección micótico/de hongos, bacterial o viral de la piel. Se considera la exclusión si: El estudiante no puede participar cómodamente en las actividades usuales El estudiante requiere mayor cuidado médico del que el personal escolar puede proveer El estudiante tiene una fiebre alta, cambios de comportamiento, llanto persistente, dificultad para respirar, tos incontrolable u otra señal sugiriendo una enfermedad severa.
