The definition used by the U.S. Public Health Service and by the Cardiac Rehabilitation/Secondary Prevention Performance Measures Writing Committee is as follows: “Cardiac rehabilitation services are comprehensive, long-term programs involving medical evaluation, prescribed exercise, cardiac risk factor modification, education, and counselling. These programs are designed to limit the physiologic and psychological effects of cardiac illness, reduce the risk for sudden death or re-infarction, control cardiac symptoms, stabilize or reverse the atherosclerotic process, and enhance the psychosocial and vocational status of selected patients.” AACVPR/ACC/AHA 2007 Performance Measures on Cardiac Rehabilitation for Referral to and Delivery of Cardiac Rehabilitation/Secondary Prevention Services Circulation 2007;116;1611-1642
AHA/AACVPR Scientific Statement
Cardiac rehabilitation/secondary prevention programs are recognized as integral to the comprehensive care of patients with cardiovascular disease and as such are recommended as useful and effective (Class I-A) by the American Heart Association (AHA) and the American College of Cardiology in the treatment of patients with coronary artery disease and chronic heart failure.

Benefit of exercise therapy for systolic heart failure in relation to disease severity and etiology-findings from the Heart Failure and A Controlled Trial Investigating Outcomes of Exercise Training study. This post hoc analysis of the HF-ACTION cohort explores the primary and secondary results of the HF-ACTION study by etiology and severity of illness.
CONCLUSION: Consideration of symptomatic (New York Heart Association classes II to IV) patients with HF with reduced LV function for participation in an exercise training program should be made independent of the cause of HF or the severity of the symptoms.

Cardiac Rehabilitation and Secondary Prevention of Coronary Heart Disease Cardioprotective Mechanisms
Furthermore, in the presence of advanced CAD, exercise training has been shown to induce ischemic preconditioning of the myocardium, a process by which transient myocardial ischemia during exercise enhances tolerance of the myocardium to subsequent more prolonged ischemic stress, thereby reducing myocardial damage and the risk of potentially lethal ventricular tachyarrhythmias (Circulation. 2005;111:369-376.)

Intra-arterial blood pressure measurements in cardiac patients have demonstrated that during low-intensity resistance training [40–60% maximum voluntary contraction (MVC)] with 15–20 repetitions, only modest elevations in blood pressure are revealed, similar to those seen during moderate endurance training.
